摘要
目的:探讨门冬胰岛素联合二甲双胍治疗妊娠期糖尿病患者的疗效及对患者血清胱抑素C(cystatin c,Cys C)与同型半胱氨酸(homocysteine,Hcy)水平的影响。方法:以本院2017年9月—2018年9月收治的84例妊娠期糖尿病患者为研究对象,按随机数字表法分为两组,各42例,对照组以门冬胰岛素治疗,试验组予以门冬胰岛素联合二甲双胍治疗,比较两组疗效及血清Cys C、Hcy水平。结果:治疗后,试验组Cys C、Hcy低于对照组,差异有统计学意义(P<0.05);治疗后,试验组胰岛β功能指数(homeostasis model assessment-β,HOMA-β)、胰岛素敏感指数(homeostasis model assessment of sensitive index,HOMASI)较对照组高,胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)较对照组低,差异有统计学意义(P<0.05);治疗后,试验组脐部血流阻力指数(resistance Index,RI)、搏动指数(pulsatility index,PI)、收缩期峰值流速/舒张末期流速(systolic peak?ow velocity/end diastolic velocity,S/D)均较对照组低,差异有统计学意义(P<0.05)。结论:门冬胰岛素联合二甲双胍治疗妊娠期糖尿病可有效降低Cys C、Hcy水平、改善胰岛功能、降低脐部血液阻力,进而达到改善妊娠结局的效果。
Objective:To investigate the efficacy of insulin aspart and metformin in the treatment of patients with gestational diabetes mellitus and its effect on the levels of serum cystatin C(Cys C)and homocysteine(Hcy).Methods:84 patients with gestational diabetes mellitus admitted to our hospital from September 2017 to September 2018 were used as subjects investigated,and they were randomly divided into two groups:a control group and an experimental group,42 cases each.The patients in the control group were treated with insulin aspart,and the patients in the experimental group were treated with insulin aspart and metformin.The efficacy and serum Cys C and Hcy levels were compared between the two groups.Results:After treatment,the levels of Cys C and Hcy in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the homeostasis model assessment-β(HOMA-β)and homeostasis model assessment of sensitive index(HOMA-SI)in the experimental group were higher than those in the control group,and the homeostasis model assessment of insulin resistance(HOMA-IR)was lower than that in the control group,with statistically significant differences(P<0.05).After treatment,the umbilical blood?ow resistance index(RI),pulsatility index(PI)and systolic peak flow velocity/end diastolic velocity(S/D)in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Insulin aspart combined with metformin can effectively reduce Cys C and Hcy levels,improve islet function and reduceumbilical blood resistance in the treatment of gestational diabetes mellitus to improve the outcome of pregnancy.
