摘要
目的:观察血府逐瘀汤治疗晚期肺癌瘀血阻络证癌痛患者的临床疗效。方法:采用前瞻性随机对照的临床设计,将240例晚期肺癌瘀血阻络证癌痛患者随机分为治疗组和对照组,两组均按照规范化止痛原则治疗,治疗组在此基础上,加用血府逐瘀汤,连续治疗21 d。比较两组患者等效口服吗啡量、疼痛程度评分、癌痛疗效评价、中医证候积分以及生活质量差异。结果:最终纳入可评价患者228例,治疗组115例,对照组113例。两组患者在性别、年龄、接受肿瘤综合治疗、癌痛辅助治疗、疼痛部位等指标均无统计学差异(P>0.05)。两组患者在第1个24 h内等效口服吗啡量无统计学差异,与治疗前比较,两组患者等效口服吗啡量均显著下降(P<0.01);治疗后,治疗组等效口服吗啡量明显少于对照组(P<0.05);与治疗前比较,治疗后两组患者的最重、最轻以及平均疼痛程度均显著减轻(P<0.01);治疗组有效率94.8%,明显高于对照组的86.7%(P<0.05);治疗组中医证候积分改善率95.7%,明显高于对照组的76.1%(P<0.05);与治疗前比较,两组患者经治疗后生活质量均有所改善(P<0.05),治疗后两组之间生活质量无差异(P>0.05)。结果:血府逐瘀汤联合规范化止痛治疗可缓解晚期肺癌瘀血阻络证患者疼痛,改善患者不适症状,且显著降低患者阿片类止痛药物摄入量。
Objective:To observe the effect of Xuefu Zhuyu Decoction(血府逐瘀汤)on the pain from advanced lung cancer with Yuxuezuluo(瘀血阻络)syndrome.Methods:A prospective randomized controlled design was adopted.A total of 240 patients with pain from advanced lung cancer of Yuxuezuluo syndrome were randomized into treatment group and control group.Both groups were treated according to the standardized analgesic principle.On this basis,the treatment group was added with Xuefu Zhuyu Decoction(21 days).The oral morphine equivalent,pain degree score,efficacy on cancer pain,TCM syndrome score,and quality of life were compared between the two groups.Results:Finally,228 patients were included,115 in the treatment group and 113 in the control group.There was no significant difference in gender,age,comprehensive tumor treatment,adjuvant treatment of cancer pain,and pain location between the two groups(P>0.05).Within the 1 st 24 h,the two groups showed no significant difference in oral morphine equivalent,and the oral morphine equivalent was decreased in both groups after treatment as compared with that before treatment(P<0.01).After treatment,the oral morphine equivalent in the treatment group was less than that in the control group(P<0.05).The severest,mildest,and average pain were aleviated in both groups after treatment as compared with those before treatment(P<0.01).The effective rate in the treatment group was 94.8%,higher than the 86.7%in the control group(P<0.05).The improvement of TCM syndrome score in the treatment group(95.7%)was better than that in the control group(76.1%)(P<0.05).The quality of life was improved in both groups after treatment(P<0.05),and no difference was observed between the two groups after treatment(P>0.05).Conclusion:Xuefu Zhuyu Decoction combined with standardized analgesic treatment can alleviate the pain of patients with advanced lung cancer of Yuxuezuluo syndrome,relieve related symptoms,and help significantly reduce the doasage of opioid painkillers.
