摘要
目的通过分析儿童呼吸道感染EB病毒(EBV)感染患者血清多种抗体检测特点得出不同感染情况下的临床特征相关性。方法焦作市妇幼保健院儿科呼吸病区于2019年1月至2019年12月收治确诊为EB病毒感染导致的传染性单核细胞增多症(IM)的480例患儿:采用化学发光法检测血浆EBV四项抗体(抗VCA-IgG、抗VCA-IgM、抗NA-IgG、抗EA-IgG),分析传染性单核细胞增多症中EBV多抗体检测结果特点,并结合患儿临床资料进行统计学分析。结果以季节为统计要素分析结果,显示0~3岁组就诊患儿发生IM多发于冬季(15.4%),少见夏季(9.2%)。4~6岁就诊患儿发生以春、冬季最高。7~12岁就诊患儿以春季最高(17.1%)。多抗体检测分析中0~3岁组患儿抗衣壳抗原IgM(VCA-IgM)阳性检出率0~3岁组就诊患儿最高,达21.6%(χ^2=6.245,P=0.044)。结论传染性单核细胞增多症儿童阶段易感人群为0~3岁婴幼儿,且0~3岁组VCA-IgM阳性检出率最高。抗VCA-IgG,抗NA-IgG抗体阳性检出率随着年龄增长而升高,但IgG类抗体中EA-IgG在各年龄组阳性检出率均为最低。按照不同季节进行统计分析,各年龄段的儿童EBV感染都易发生在春季。
Objective:To obtain the correlation of clinical characteristics under different infection situations by analyzing the detection characteristics of multiple antibodies in the sera of patients with EBV infection in children with respiratory tract infection.Methods:From January 2019 to December 2019,480 children diagnosed with infectious mononucleosis(IM)caused by Epstein-Barr virus infection were admitted to the Pediatric Respiratory Department of Jiaozuo Maternal and Child Health Hospital:Chemiluminescence detection was used Plasma EBV four antibodies(anti-VCA-IgG,anti-VCA-IgM,anti-NA-IgG,anti-EA-IgG),analyze the characteristics of EBV multi-antibody detection results in infectious mononucleosis,and combine the clinical data of children Statistical analysis.Results:The analysis results based on the seasonal statistical factors showed that the incidence of IM occurred in children in the 0-3 year old group more frequently in winter(15.4%),and rarely in summer(9.2%).The incidence of children aged 4 to 6 years was highest in spring and winter.The highest rate among children aged 7-12 years old was in spring(17.1%).In the multi-antibody detection analysis,the positive detection rate of anti-capsid antigen IgM(VCA-IgM)in the 0-3 year old group was the highest in the 0-3 year old group,reaching 21.6%(χ~2=6.245,P=0.044).Conclusion:The susceptible population of infectious mononucleosis in childhood is 0-3 years old infants,and the positive detection rate of VCA-IgM is the highest in the 0-3 year old group.The positive detection rate of anti-VCA-IgG and anti-NA-IgG antibodies increased with age,but the positive detection rate of EA-IgG among IgG antibodies was the lowest in each age group.According to the statistical analysis of different seasons,children of all ages are prone to EBV infection in spring.
