摘要
目的建立本地区妊娠期女性在孕早、中、晚期甲状腺激素正常参考标准,并分析孕期甲状腺激素变化规律。方法选取2017年3月至2019年3月在蕲春县人民医院、第二人民医院、第三人民医院及妇幼保健院产检的325名妊娠早期孕妇作为本次研究组,在孕早、中、晚期抽血送检,选取同期体检的280名非妊娠期女性作为对照组,抽检一次,使用化学发光免疫检测两组患者甲状腺激素水平,双人双机录入检测结果核对无误后进行分析。结果(1)本次共有307例孕妇最终进入研究组,本地区孕妇孕早、中、晚期血清TSH、FT3、FT4参考区间分别为(0.23~5.38)mIU/L,(0.88~6.18)mIU/L,(1.59~5.76)mIU/L;(2.56~6.34)pmol/L,(3.06~6.00)pmol/L,(2.86~5.17)pmol/L;(12.12~19.35)pmol/L,(11.24~18.52)pmol/L,(9.72~16.20)pmol/L。(2)随着孕周增加,孕早期及孕中期TSH值降低,在孕晚期出现回升;FT3值逐渐下降,均低于非妊娠期;FT4值孕早期升高,随着孕周增加逐渐下降,孕晚期最低。结论本研究建立了蕲春县妊娠期女性在不同孕期甲状腺激素正常参考标准,并分析了甲状腺激素变化规律,为预防妊娠期甲状腺疾病漏诊及误诊提供依据。
Objective:To establish the normal reference standard of thyroid hormone for pregnant women in the early,middle and late pregnancy,and to analyze the changes of thyroid hormone during pregnancy.Methods:From March 2017 to March 2019,325 pregnant women in early pregnancy were selected as the study group in the obstetric examination of Qichun County People′s Hospital,the second people′s Hospital,the third people′s Hospital and the maternal and child health care hospital.Blood samples were taken at the early,middle and late stages of pregnancy.280 non pregnant women who had physical examination at the same time were selected as the control group,and blood samples were taken once.The levels of thyroid hormone in the two groups were detected by chemiluminescent immunoassay and analyzed after the results were checked by two persons and two computers.Results:(1)A total of 307 pregnant women were admitted to the study group.The reference intervals of serum TSH,FT3 and FT4 were(0.23~5.38)mIU/L,(0.88~6.18)mIU/L and(1.59~5.76)mIU/L in the early,middle and late stages of pregnancy,respectively.(2.56~6.34)pmol/L,(3.06~6.00)pmol/L,(2.86~5.17)pmol/L.(12.12~19.35)pmol/L,(11.24~18.52)pmol/L,(9.72~16.20)pmol/L.(2)With the increase of gestational weeks,TSH decreased in the first and second trimesters and increased in the third trimester.The FT3 value decreased gradually and was lower than that of non-gestational period.The FT4 value increased in the first trimester and decreased gradually with the increase of gestational age,and the lowest in the third trimester.Conclusion:In this study,the normal reference standard of thyroid hormone in different pregnant women in Qichun county was established,and the rule of thyroid hormone change was analyzed to provide basis for the prevention of missed diagnosis and misdiagnosis of thyroid diseases in pregnancy.
