摘要
目的探讨血清胱抑素C(Cysc)和内脂素(Visfatin)在妊娠期糖尿病(GDM)早期肾损伤的诊断价值。方法选取2018年9月~2019年9月期间深圳市龙华区人民医院孕24~28w产检的90例妊娠期糖尿病患者为观察组,选取同期30例正常妊娠妇女作为对照组。以尿白蛋白排泄率(UAER)为标准,将观察组分为正常蛋白尿A组(31例,UAER<30μg/min),微量蛋白尿B组(34例,30μg/min<UAER<300μg/min),大量蛋白尿C组(25例,UAER>300μg/min),检测各组血清Cysc和Visfatin及尿素氮和肌酐水平,受试者工作特征曲线(ROC)分析Cysc和Visfatin诊断GDM早期肾损伤的价值。结果血清Cysc和Visfatin水平C组>B组>A组(P均<0.05),Cysc和Visfatin阳性检出率蛋白尿组>正常蛋白尿组(P<0.05),Cysc和Visfatin及联合诊断GDM早期肾损伤的曲线下面积(AUC)分别为0.763(95%CI:0.637~0.889)、0.770(95%CI:0.650~0.891)、0.864(95%CI:0.726~0.950),Cysc最佳诊断临界值为1.15 mg/L,Visfatin最佳诊断临界值为13.2μg/L。结论联合检测血清Cysc和Visfatin水平有助于早期诊断GDM肾损伤,早干预,减少母儿并发症。
Objective:To investigate the diagnostic value of early renal damage in gestational diabetes(GDM)of serum cystic suppressor C(Cysc)and neslipidin(Visfatin).Methods:Selected 90 cases of gestational diabetes from 24 to 28 weeks of pregnancy at Longhua District People’s Hospital in Shenzhen from September 2018 to September 2019,and 30 normal pregnant women as the control group.Based on unoicint excretion rate(UAER),the observation group was divided into normal proteinuria Group A(31 cases,UAER<30μg/min),microproteinuinuinia B group(34 cases,30μg/min<UAER<300μg/min),a large number of proteinuria C groups(25 cases,UAER>300μg/min),detection of each group of serum Cysc and Visfatin and urea nitrogen and creatinine levels,subject working characteristiccurve(ROC)analysis of cysc and Visfatin early renal damage value.Results:Serum Cysc and Visfatin levels in group C was more than in Group B,Group B was more than Group A(P<0.05),Positive detection rate of Cysc and Visfatin in protein urination group was more than normal protein urination group(P<0.05),Cysc and Visfatin and joint diagnosis of GDM early renal damage under the curve area(AUC)was 0.763(95%CI:0.637~0.889),0.770(95%CI:0.650~0.891),0.864(95%CI:0.726~0.950),Cysc best diagnostic threshold is 1.15 mg/L,Visfatin best diagnosis value is 13.2μg/L.Conclusion:Joint detection of serum Cyc and Visfatin levels can help to diagnose GDM early renal damage,through intervene early and reduce maternal and child complications.
