摘要
慢性萎缩性胃炎是消化系统常见的一种以固有腺体萎缩、减少导致胃黏膜变薄,黏膜肌层增厚的慢性疾病,常伴有炎症及肠上皮化生和不典型增生,被列为胃癌的癌前病变疾病。大多数学者认为其发病机制与幽门螺杆菌(Helicobacter pylori,Hp)感染、胆汁反流、血管活性因子和细胞因子改变以及自身免疫因素等相关,其中Hp感染是主要致病因素。目前西医尚无有效治疗方法逆转这种炎–癌转变,而中医药治疗能更好地改善患者的症状,同时还能有效阻断或逆转黏膜病变,及时有效阻断慢性萎缩性胃炎癌变,具有独特优势,是当前的研究热点。中医药在治疗慢性萎缩性胃炎方面取得了显著进展,其辨证分型、中成药、中西医结合治法以及针灸治疗等体现了中医药安全有效、治疗手段多样的特点。鉴于慢性萎缩性胃炎错综复杂的病机特点,中医理论以辨证论治为主流,针对慢性萎缩性胃炎病机及证候表现采取预防措施。中成药效果好,不良反应少,便于携带。中西医结合治疗患者的治疗效果相比单纯西医治疗明显有了更大的提升,其临床表现和生命质量也有所改善,且中西医治疗是更符合中国国情的治疗模式。文章通过对近年来中医药治疗本病的病因病机、辨证论治、中成药、中西医结合治疗及针灸治疗作用机制进行探讨,梳理相关文献并对本病的中医药治疗进展作出综述。
Chronic atrophic gastritis is a common chronic disease of digestive system with atrophy and reduction of inherent glands,resulting in gastric mucosa thinning and mucosal muscularity thickening.It is often accompanied by inflammation,intestinal metaplasia and atypical hyperplasia,and is listed as a precancerous disease of gastric cancer.Most scholars believe that the pathogenesis is related to Hp infection,bile reflux,changes of vasoactive factors and cytokines,and autoimmune factors,among which Hp infection is the main pathogenic factor.At present,there is no effective treatment in Western medicine to reverse this inflammatory-cancer transformation,while traditional Chinese medicine treatment can better relieve the symptoms of patients,but also effectively block or reverse mucosal lesions,timely and effectively block the cancer of chronic atrophic gastritis with unique advantages,and it is the current research hotspot.TCM has made remarkable progress in the treatment of chronic atrophic gastritis,and its safe and effective treatment and diversified treatment methods are reflected in the aspects of syndrome differentiation,Chinese patent medicine,the integrative medicine treatment and acupuncture treatment.In view of the complex pathogenesis of chronic atrophic gastritis,TCM theory takes syndrome differentiation as the mainstream and takes preventive measures against the pathogenesis and symptoms of chronic atrophic gastritis.Chinese patent medicine has good effect,fewer adverse reactions and is easy to carry.Compared with simple Western medicine treatment,the therapeutic effect of the integrative medicine treatment has been significantly improved,and its clinical manifestations and quality of life have also been improved,and the treatment of the integrative medicine treatment is more in line with China’s national conditions of the treatment mode.In this paper,the etiology and pathogenesis,syndrome differentiation and treatment,Chinese patent medicine,the integrative medicine treatment and mechanism of TCM moxibustion treatment in recent years are discussed,and the progress of TCM treatment is summarized by combing relevant literature.
