摘要
目的:运用数据挖掘总结2011年―2021年苏道刚治疗胃炎的中药用药规律。方法:提取山东中医药大学附属医院的医院信息系统(Hospital Information System,HIS)中胃炎数据,其间共收录苏道刚治疗胃炎患者的271个治疗处方。运用Excel 2010建立苏道刚中药治疗胃炎处方用药规律数据库并进行分类整理,通过SPSS Modeler 18.0、SPSS 26.0软件进行关联规则及聚类分析。结果:研究共纳入方剂271首,涉及中药121味,药物使用频次共计3804次。结果显示党参、白术、甘草、白及、白芍为使用频次居前5位的中药,其中,党参使用频次最高,达到230次。通过关联规则分析,得到支持度≥45%,100%≥置信度≥80%的关联规则共27条,支持度最高的药对组合为党参-白术,通过聚类分析共得出8个有效聚类,分别为黄芪-当归、海螵蛸-浙贝母、赤芍-柴胡、砂仁-醋香附、半枝莲-山药、醋延胡索-白芍、炒麦芽-麸神曲、茯苓-白及;以及13个核心中药组合,分别为白术、木香、菟丝子、麸炒枳实、黄连各为一类,其余的中药组合为半枝莲-白花蛇舌草-山药-醋鸡内金、醋香附-砂仁、白芍-醋延胡索、白及-茯苓-党参、麸神曲-炒麦芽、浙贝母-海螵蛸、柴胡-赤芍-甘草、当归-黄芪。结论:苏道刚治疗胃炎过程中,以辨证论治、随证加减为总则,根据疾病虚实,以扶正为主,治以健脾和胃;祛邪为辅,用以疏肝理气,消食化积。
Objective:To summarize the medication rule of SU Daogang in treating gastritis based on data mining from 2011 to 2021.Methods:Gastritis data was extracted from the hospital information system(HIS)of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine,during which 271 treatment prescriptions of SU Daogang for gastritis patients were included.Excel 2010 was used to establish the prescription rule database of SU Daogang’s TCM treatment of gastritis,and the classification and sorting was carried out.SPSS Modeler 18.0 and SPSS 26.0 were used for association rules and cluster analysis.Results:The study included 271 prescriptions,involving 121 TCM medicines,the frequency of drug use was of a total of 3804 times.The results showed that Dangshen(Radix Codonopsis),Baizhu(Rhizoma Atractylodis Macrocephalae),Gancao(Radix Glycyrrhizae),Baiji(Rhizoma Bletillae),Baishao(Radix Paeoniae Alba)were the top 5 frequently used herbs,with Dangshen being the most frequently used,which was for 230 times.Through association rule analysis,a total of 27 association rules with support≥45%and100%≥confidence≥80%were obtained,the drug pair combination with the highest support was obtained as Dangshen-Baizhu,and through cluster analysis,a total of 8 effective clusters were identified as Huangqi(Radix Astragali seu Hedysari)-Danggui(Radix Angelicae Sinensis),Haipiaoxiao(Os Sepiellae seu Sepiae)-Zhebeimu(Bulbus Fritillariae Thunbergii),Chishao(Radix Paeoniae Rubra)-Chaihu(Radix Bupleuri),Sharen(Fructus Amomi Villosi)-Cu Xiangfu(Rhizoma Cyperi),Banzhilian(Herba Scutellariae Barbatae)-Shanyao(Rhizoma Dioscoreae),Cu Yanhusuo(Rhizoma Corydalis)-Baishao,Chao Maiya(Fructus Hordei Germinatus)-Fu Shenqu(Massa Medicata Fermentata),Fuling(Poria)-Baiji,and the 13 core herbal combinations were Baizhu,Muxiang(Radix Aucklandiae),Tusizi(Semen Cuscutae),Fuchao Zhishi(Fructus Aurantii Immaturus),Huanglian(Rhizoma Coptidis),and the other categories were Banzhilian-Baihuasheshecao(Herba Hedyotis)-Shanyao-Cu Jineijin(Endothelium Corneum Gigeriae Galli),Cu Xiangfu-Sharen,Baishao-Cu Yanhusuo,Baiji-Fuling-Dangshen,Fu Shenqu-Chao Maiya,Zhebeimu-Haipiaoxiao,Chaihu-Chishao-Gancao,Danggui-Huangqi.Conclusion:In the treatment of gastritis,SU Daogang takes syndrome differentiation as the general principle,and the general principle of modifying prescriptions according to the symptoms,based on deficiency and excess property of the disease,the main treatment is to support the healthy Qi(气)and strengthen the spleen and stomach,which is combined with eliminating the evil,so as to soothe the liver and regulate Qi,and eliminate food accumulation and stagnation.
