摘要
目的:系统评价乌梅丸加减联合中药灌肠治疗溃疡性结肠炎(Ulcerative Colitis,UC)的临床疗效及安全性。方法:按Cochrane系统评价方法,计算机检索万方数据知识服务、The Cochrane Library、中国知网、中国生物医学文献数据库、Embase、维普期刊平台、美国国立医学图书馆资源,检索时间限定为自数据库创建至2022年2月1日,查找采用乌梅丸加减联合中药灌肠治疗UC的随机对照试验文献,采用国际广泛权威认可的Cochrane偏倚风险工具进行文献的方法学质量评价,对符合要求的文献研究样本用Rev Man 5.4软件合并计算,并进行异质性评价,采用逐一排除法进行敏感性分析,发表漏斗图进行偏倚评估。结果:共纳入9项研究,Meta分析提示,试验组在提高总有效率、治愈率以及降低不良反应发生率、复发率方面优于对照组(P<0.05);按照治疗方式的不同进行亚组分析,结果提示不同的治疗方式对研究结果的影响不存在明显差异(P>0.05)。试验组在降低中医症状腹痛、腹泻、体温、红细胞沉降率方面显著优于对照组(P<0.05),但两组在降低中医症状脓血便积分方面的差异无统计学意义(P>0.05)。结论:Meta分析结果显示,乌梅丸加减联合中药灌肠疗效胜于西药,在临床上可用于推广以及使用。
Objective:To systematically evaluate the efficacy and safety of Wumei Wan(乌梅丸)plus Chinese materia medica enema on ulcerative colitis.Methods:Databases of Wanfang,the Cochrane Library,CNKI,CBM,Embase,VIP,Pub Med were searched.The retrieval time is limited from database initial stage to Februaryl,2022;randomized controlled trials of Wumei Wan plus Chinese materia medica enema on ulcerative colitis were found.The internationally widely recognized Cochrane bias risk tool was used to evaluate the methodological quality of the included literature.The eligible literature research samples were accounted by Rev Man 5.4 software,and the heterogeneity was evaluated.The sensitivity analysis was conducted by one by one exclusion method,and the funnel chart was established for a bias evaluation.Results:A total of 9 studies were included.The Meta analysis suggested that the test group was superior to the control group in improving the total efficiency and cure rate,and reducing the incidence of adverse reactions and recurrence rate(P<0.05).The subgroup analysis was conducted according to different treatment methods,and the results indicated that there was no significant difference in the infuence of different treatment methods on the study results(P>0.05).The test group was significantly better than the control group in reducing TCM symptoms of abdominal pain,diarrhea,body temperature and erythrocyte sedimentation rate(P<0.05),but there was no significant difference between the two groups in reducing TCM symptom score of stool with blood and pus(P>0.05).Conclusion:The curative effect of Wumei Wan plus Chinese materia medica enema was better than that of Western medicine,which could be used for clinical promotion and treatment.
