摘要
卒中后抑郁属于一种情感和认知功能障碍,是脑血管疾病常见的并发症之一,主要临床症状有情绪低落、兴趣减退,甚至出现自杀倾向等,给患者及其家庭带来沉重的精神负担和经济压力。目前西方医学对其发病机制尚未形成一个统一的认识,临床常给予5-羟色胺再摄取抑制剂为主的药物干预,治疗方法单一,治疗周期较长,还容易引起胃肠道反应,加重肝肾代谢负担等,效果欠佳。相比之下,中医药可以从多靶点、多角度干预卒中后抑郁,具有经济实惠和不良反应少等优势,近年来得到了患者的普遍认可。现笔者从卒中后抑郁的病因病机、治则治法和用药疗效方面总结近年来中药在该领域的研究进展,以期为临床提供一定的参考。
Post-stroke depression is a kind of emotional and cognitive dysfunction,which is one of the the common complications of cerebrovascular disease,The clinical symptoms are mainly depressed,loss of interest,and even suicide tendency,which bring heavy mental burden and economic pressure to patients and their families.At present,a unified conclusion has not been gained on the pathogenesis of post-stroke depression in Western medicine.In clinical practice,drug intervention based on 5-hydroxytryptamine reuptake inhibitor is often given,but the treatment method is single,and the treatment cycle is long.At the same time,it is easy to cause gastrointestinal reactions and aggravate the metabolic burden on the liver and kidney,and the effect is not good.In contrast,TCM can intervene in post-stroke depression from multiple targets and perspectives,has the advantages of economic benefits and fewer adverse reactions,and has been widely recognized by patients in clinical treatment in recent years.Now the author summarizes the research progress of TCM in the treatment of post-stroke depression in recent years from the perspectives of etiology,pathogenesis,therapeutic principles and therapeutic effects,with a prospect of providing a certain reference for the clinical practice.
