临床护士人际冲突处理型态、职业疲溃感与离职意愿的相关性分析 被引量:10

Correlation study of interpersonal conflict management patterns,occupational fatigue and turnover intention of clinical nurses

摘要 目的探讨临床护士人际冲突处理型态、职业疲溃感与离职意愿的相关性。方法采用便利抽样法于2019年4—5月对天津市某三级甲等医院的345名临床护士采用护士人际冲突处理型态量表、护士职业疲溃感量表和离职意愿量表进行问卷调查,并对数据进行分析。结果护患冲突是临床护士工作过程中面临的主要冲突,占比为77.4%;临床护士人际冲突处理型态得分最高的维度是整合(12.33±3.02);职业疲溃感中情感衰竭、去人格化和个人成就感维度得分分别为21.54±8.36、5.14±3.28和28.32±6.25。临床护士的离职意愿处于中高水平(43.85±5.27),护士人际冲突处理方式中整合与让步维度与职业疲溃感和离职意愿呈正相关,逃避与支配维度与职业疲溃感和离职意愿呈负相关;临床护士职业疲溃感与离职意愿各维度呈正相关(P<0.01)。离职意愿对职业疲溃感中的情感衰竭及去人格化维度有正向预测作用。结论护理管理人员应该注重临床护士人际冲突不同处理型态所产生的后果,引导护士正确处理人际冲突,降低职业疲溃感,降低护士离职意愿,以稳定护士队伍,保证护理质量。 Objective To study the correlation between interpersonal conflict management patterns,occupational fatigue and turnover intention of clinical nurses.Methods Using the convenience sampling method,345 clinical nurses in a grade-A tertiary hospital in Tianjin from April to May 2019 were investigated with the nurses’interpersonal conflict management type scale,the nurse occupational fatigue scale,and the turnover intention scale.And the data were analyzed.Results The conflict between nurses and patients was the main conflict in the process of clinical nurses’work,accounting for 77.4%.The score of integration dimension was the highest(12.33±3.02)in the clinical nurses’interpersonal conflict management pattern.The score of emotional exhaustion,depersonalization and personal accomplishment dimensions in occupational burnout was 21.54±8.36,5.14±3.28 and28.32±6.25.The clinical nurses’turnover intention was at a medium-high le vel(43.85±5.27).The integration and concession dimensions of nurses’interpersonal conflict management patterns were positively correlated with occupational fatigue and turnover intention,while the escape and dominance dimensions were negatively correlated with occupational fatigue and turnover intention.There were positive correlations between the occupational fatigue of nurses and the dimensions of turnover intention(P<0.01).The turnover intention had a positive predictive effect on the emotional exhaustion and depersonalization dimension of occupational fatigue.Conclusion Nursing managers should pay attention to the consequences of different interpersonal conflict management patterns of clinical nurses,guide nurses to correctly handle interpersonal conflicts,reduce occupational fatigue and nurses’turnover intention,to stabilize the nurse team and ensure the quality of care.
