摘要
目的观察丹栀逍遥散联合微波消融治疗育龄期妇女甲状腺功能亢进症(Graves病)的临床疗效,评估其安全性。方法选取2020年1月—2020年10月佛山市中医院内分泌科收治的女性育龄期Graves病患者68例为研究对象,用随机数字表法分为观察组34例、对照组34例,对照组接受微波消融治疗,观察组在此基础上配合丹栀逍遥散治疗。比较两组治疗后3个月、6个月临床疗效及中医证候积分改善情况,观察促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺激素(FT4)水平变化情况,并评估治疗安全性。结果观察组均手术成功,术后未出现声音嘶哑、皮下血肿、切口感染等;观察组治疗后3个月总有效率(91.17%,31/34)明显高于对照组(70.59%,24/34)(P<0.05);治疗后,两组中医证候积分较治疗前显著下降(P<0.05),两组治疗后6个月中医证候积分显著低于治疗后3个月(P<0.05),治疗后两组中医证候积分组间比较差异有统计学意义(P<0.05);治疗后,两组FT3、FT4水平较治疗前显著下降,TSH水平显著上升(P<0.05),两组治疗后6个月FT3、FT4水平显著低于治疗后3个月,TSH水平显著高于治疗后3个月(P<0.05),治疗后,两组TSH、FT3及FT4水平组间比较差异有统计学意义(P<0.05);两组不良反应发生率[2.94%(1/34)、14.71%(5/34)]比较差异无统计学意义(P>0.05)。结论丹栀逍遥散联合微波消融治疗育龄期妇女Graves病疗效确切,能有效改善甲状腺功能,安全性好。
Objective To investigate the clinical efficacy and safety of Danzhi Xiaoyao Powder(丹栀逍遥散)combined with microwave ablation in the treatment of Graves disease in women of childbearing age.Methods Sixty women of childbearing age with Graves disease in Endocrinology Department of Foshan Hospital of Traditional Chinese Medicine from January 2020 to December 2020 were enrolled and divided into two groups by random number table methods,each with 30 cases.The control group received microwave ablation,while the observation group received microwave ablation combined with Danzhi Xiaoyao Powder.And then various indexes were compared between groups,including clinical efficacy,TCM syndrome scores,thyroid stimulating hormone(TSH),free triiodothyronine(FT3)and free thyroid hormone(FT4)and safety.Results All patients in the observation group achieved successful operation with no hoarseness,subcutaneous hematoma or incision infection.The clinical efficacy after 3 months of treatment in the observation group was significantly higher than that in the control group(91.17%vs 70.59%,P<0.05),while the clinical efficacy showed no significant difference between two groups after 6 months of treatment(P>0.05).After treatment,there was a decreasing trend in TCM syndrome score in both groups(P<0.05),and the score at post-treatment 6-month was significantly lower than that at post-treatment 3-month(P<0.05).There was a decrease in levels of FT3 and FT4 and an increase in TSH level in both groups(P<0.05).Compared with the data at post-treatment 3-month,patients in both groups had significantly lower levels of FT3 and FT4 and significantly higher TSH levels(P<0.05).The adverse reaction rate showed no significant difference between two groups(2.94%vs 14.71%,P>0.05).Conclusion Application of Danzhi Xiaoyao Powder combined with microwave ablation is an effective and safe method in the treatment of Graves disease in women of childbearing age,which can effectively improve the thyroid function.
