双股高强度线加强缝合技术治疗急性髌腱断裂

Treatment of acute patellar tendon rupture with double high strength thread tension suture

摘要 目的探讨采用高强度线加强缝合技术治疗急性髌腱断裂的临床疗效。方法对我科自2015年至2018年连续治疗的18例急性髌腱断裂的患者进行回顾性研究,其中男性16例,女性2例,平均年龄32岁(28~41岁);6例为摔倒或交通伤,12例为运动损伤,其中5例患者髌尖部位曾接受皮质激素类固醇注射。所有患者均采用高强度线(强生Orthcord)做加强,断端吻合的缝合技术治疗。术后通过Caton指数、Lysholm、Kujala评估膝关节功能。结果所有患者均得到随访,平均随访时间32.7个月(25~48个月);术后关节屈曲度(145.22±3.64)°,Lyshlom评分(91.06±3.87);Kujala评分(90.06±3.44)。核磁共振检查均愈合;无高位髌骨或低位髌骨等明显并发症。结论采用高强度线加强缝合技术治疗急性髌腱断裂能够获得良好的短期疗效,手术难度不高,具有无需二次取出内固定物的优势。 Objective To explore clinic effect of treatment of double high strength thread tension suture on acute patellar tendon rupture.Methods 18 patients with acute patellar tendon rupture in our department from 2015 to2018 were reviewed.There were 16 males and 2 females.The average age was 32 years old(from 28 to 41 years old);6 cases were falls or traffic injury and 12 cases were sports injury.5 patients have received corticosteroid steroid injection into the patellar tendon once.All patients were treated with rupture end anastomosis and reinforeced by tension reduction using high strength thread(Orthcord).After the operation,Caton index,Lysholm and Kujala were used to evaluate the function of knee joint.Results All patients were followed up.The average follow-up time was32.7 months(from 25 to 48 months).After surgery,the joint flexion was 142 degrees(from 140 to 150 degrees).Lyshlom score was 98.8(from 97 to 100);Kujala score was 95.2(from 92 to 97).All rupture was healed demonstrated by MRI.No high patella was found.Conclusion Reinforeced suture technique using high strength thread(Orthcord)on acute patellar tendon rupture has aquired exact short-term curative effect and isn’t really challenging.Another advantage is that there is no need for a reoperation to remove the interanl fixtion.
