全膝关节置换术后患膝皮温、C反应蛋白和红细胞沉降率变化规律及相关性分析 被引量:1

Change pattern and correlation analysis of skin temperature of affected knee, C-reactive protein and erythrocyte sedimentation rate after total knee arthroplasty

摘要 目的分析无并发症全膝关节置换术患者术后患膝皮温、C反应蛋白(CRP)和红细胞沉降率(ESR)的变化规律及相关性,探讨其在术后感染和非感染炎症反应鉴别诊断中的意义。方法选取2021年1月至2022年1月于河北大学附属医院行单侧全膝关节置换术的36名患者为研究对象,分别于术前1天、术后1天、4天、7天、10天、14天、1月、2月、3月、6月测量双膝皮温、CRP和ESR,比较双膝皮温变化,分析双膝皮温差、CRP、ESR的变化规律,采用Spearman和Pearson法分析CRP、ESR与双膝皮温差变化的相关性。结果术后患膝皮温高于健膝皮温(P<0.05)。双膝皮温差、CRP、ESR均表现为先快速升高后缓慢降低的变化趋势,双膝皮温差于术后4天达到最大值,术后6月恢复至术前水平;CRP于术后4天达到最大值,术后1月基本恢复至正常水平;ESR于术后7天达到最大值,术后3月基本恢复至正常水平。CRP、ESR与双膝皮温差变化均呈正相关(ρ=0.979,P<0.001;r=0.773,P=0.009)。结论全膝关节置换术后双膝皮温差、CRP、ESR均较术前一定程度升高,这可能是非感染性炎症反应造成的结果。 Objective To analyze the change pattern and correlation of skin temperature of affected knee,C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)in patients with uncomplicated total knee arthroplasty,and to explore their significance in the differential diagnosis of postoperative infection and non-infection inflammation.Methods A total of 36 patients who underwent unilateral total knee arthroplasty in the Affiliated Hospital of Hebei University from January 2021 to January 2022 were selected as the research objects.Skin temperature of both knees,CRP and ESR were measured 1 day before surgery,1 day,4 days,7 days,10 days,14 days,1 month,2 months,3 months and 6 months after surgery.The changes of skin temperature of both knees were compared,and the change pattern of skin temperature difference of both knees,CRP and ESR were analyzed.Spearman and Pearson methods were used to analyze the correlation between CRP,ESR and skin temperature difference of both knees.Results The postoperative skin temperature of the affected knee was higher than that of the healthy knee(P<0.05).The skin temperature difference of both knees,CRP and ESR showed a trend of rapid increase at first and then slow decrease.The skin temperature difference of both knees reached the maximum at 4 days after operation and returned to the preoperative level at 6 months after operation.CRP reached the maximum level 4 days after operation,and basically returned to normal level 1 month after operation.ESR reached the maximum value at 7 days after operation,and basically returned to normal level at 3 months after operation.CRP and ESR were positively correlated with skin temperature difference of both knees(ρ=0.979,P<0.001;r=0.773,P=0.009).Conclusion The skin temperature difference of both knees,CRP and ESR after total knee arthroplasty were higher than those before operation,which may be the result of non-infectious inflammatory reaction.
