摘要
目的研究再生障碍性贫血(AA)病人外周血NK细胞及其受体表达的变化,探讨NK细胞在AA发病机制中的作用。方法利用流式细胞术检测28例AA病人(其中SAA 9例,NSAA 19例)外周血NK细胞功能,包括NK细胞数量,活化性受体NKG2D,抑制性受体NKG2A,穿孔素,颗粒酶β的表达,T淋巴细胞亚群及与造血指标的相关性分析,另有18例健康志愿者为对照组。结果AA病人外周血NK细胞数,NKG2D,颗粒酶表达减少,与对照组比较差异有统计学意义(P<0.05);NSAA病人活化性受体NKG2D表达与血红蛋白含量(r=0.979,P=0.007)、血小板数(r=0.943,P=0.018)有相关性;颗粒酶表达与血红蛋白含量(r=0.942,P=0.018)、网织红细胞数(r=0.906,P=0.029)有相关性;SAA病人的穿孔素表达与网织红细胞数有相关性(r=0.907,P=0.046);NSAA病人NK细胞的活化性受体NKG2D表达与CD3+T淋巴细胞有相关性(r=0.984,P=0.005);NK细胞的胞浆颗粒酶表达与CD3+T淋巴细胞(r=0.865,P=0.042),CD8+T淋巴细胞(r=0.928,P=0.022)有相关性;SAA病人NK细胞数与CD4+T淋巴细胞有相关性(r=0.988,P=0.006)。结论AA病人外周血NK细胞数及受体表达减少,免疫异常,直接或间接参与了造血调控,与骨髓造血衰竭相关,是再生障碍性贫血发病的重要环节。
Objective To analyze the functional changes of natural killer(NK)cell in peripheral blood of AA patients so as to explore the role of NK cell in the pathogenesis of AA.Methods The function of NK cells in peripheral blood of 28 AA patients(9 cases of SAA and 19 cases of NSAA)was detected by flow cytometry,including the number of NK cells,the expression of active receptor NKG2D,inhibitory receptor NKG2A,perforin,granzyme,T lymphocyte subsets and their correlation with hematopoietic indexes.Another 18 healthy volunteers were included in the control group.Results The number of NK cells,NKG2D and granzyme expression in peripheral blood of AA patients were significantly decreased compared with those of normal control group(P<0.05).The expression of activated receptor NKG2D was correlated with hemoglobin(r=0.979,P=0.007)and platelet count(r=0.943,P=0.018)in NSAA patients.Granzyme expression was correlated with Hb(r=0.942,P=0.018)and reticulocyte(r=0.906,P=0.029).The expression of perforin in SAA patients was correlated with reticulocyte(r=0.907,P=0.046).The expression of NKG2D was correlated with CD3+T lymphocytes in NSAA patients(r=0.984,P=0.005).The expression of cytoplasmic granzyme in NK cells was correlated with CD3+T lymphocytes(r=0.865,P=0.042)and CD8+T lymphocytes(r=0.928,P=0.022).There was a correlation between NK cell and CD4+T lymphocytes in SAA patients(r=0.988,P=0.006).Conclusion The expression of NK cells and receptors in peripheral blood of AA patients are decreased,and the immune abnormalities are directly or indirectly involved in the regulation of hematopoiesis and is related to bone marrow hematopoietic failure,which is an important link in the pathogenesis of aplastic anemia.
