基于锥形束CT的骨性Ⅰ类伴安氏Ⅰ类及安氏Ⅱ类错[牙合]畸形患者颞下颌关节窝形态分析 被引量:4

A morphological analysis of glenoid fossa in skeletal Class Ⅰ patients with Angle Class Ⅰ and Angle Class Ⅱ malocclusions based on cone-beam CT

摘要 目的应用锥形束CT分析骨性Ⅰ类伴安氏Ⅰ类及安氏Ⅱ类错[牙合]畸形患者颞下颌关节窝形态差异。方法收集2019年1月至2020年12月于首都医科大学附属北京友谊医院口腔科就诊的78例骨性Ⅰ类成年患者的双侧颞下颌关节锥形束CT影像资料进行回顾性分析,并根据磨牙关系及上前牙倾斜度分为安氏Ⅰ类组、安氏Ⅱ1类组及安氏Ⅱ类组。对3组颞下颌关节窝与髁突的角度和线性指标进行测量评估。结果(1)各组颞下颌关节窝和髁突测量指标总的比较,除关节结节高度(AEH)外,差异均具有统计学意义(均P<0.05)。其中,安氏Ⅱ类组和安氏Ⅱ1类组的关节结节后斜面斜度(AEI-BFL和AEI-TRL)均较安氏Ⅰ类组大(均P<0.05)。安氏Ⅱ类组的关节窝深度(GFD)大于安氏Ⅰ类组,关节窝长度(GFW)、关节窝长深比(GFW/GFD)、髁突角(CA)及髁突间角(IA)均小于安氏Ⅱ1类组和安氏Ⅰ类组(均P<0.05)。(2)除了安氏Ⅱ类组左右侧CA差异具有统计学意义(P<0.05)以外,各组左右侧颞下颌关节的其他测量指标差异均无统计学意义(均P>0.05)。(3)除了安氏Ⅱ1类组女性患者的GFW/GFD大于男性患者(P<0.05)以外,各组不同性别患者其他颞下颌关节窝测量指标比较,差异均无统计学意义(均P>0.05)。(4)各组相同性别患者的AEI-BFL、AEI-TRL及GFW/GFD比较,差异均具有统计学意义(均P<0.05)。此外,各组女性患者的GFW、CA及IA比较,差异也具有统计学意义(均P<0.05)。结论骨性Ⅰ类伴安氏Ⅰ类及安氏Ⅱ类错[牙合]畸形患者的颞下颌关节窝形态存在差异,但其两侧关节窝形态的对称性良好;且性别因素对骨性Ⅰ类伴同种牙性错[牙合]畸形患者颞下颌关节窝的形态发育影响较小,而不同错[牙合]畸形女性患者的颞下颌关节形态差异要较男性患者明显。 Objective To evaluate the morphological differences of the glenoid fossa in skeletal ClassⅠpatients with Angle ClassⅠand Angle ClassⅡmalocclusions based on cone-beam CT(CBCT).Methods The CBCT images of bilateral temporomandibular joints(TMJs)in 78 patients with skeletal ClassⅠwere selected,who were treated in Department of Stomatology,Beijing Friendship Hospital,Capital Medical University from January 2019 to December 2020.The patients were divided into ClassⅠ,ClassⅡ~1,and ClassⅡ~2 according to the molar relationship and retroclination of the maxillary incisors.Angular and linear measurements of the glenoid fossa were evaluated among the 3 groups.Results(1)The intergroup comparisons showed statistical differences for all morphological measurements except for the height of articular eminence(AEH)(P<0.05).In detail,the articular eminence inclination(AEI)in the ClassⅡ~1 and the ClassⅡ~2 groups were larger than those in the ClassⅠgroup(P<0.05).The depth of the glenoid fossa(GFD)of the ClassⅡ~2 group was larger than that of the ClassⅠgroup,whereas the width of the glenoid fossa(GFW),the ratio of the GFW to the GFD(GFW/GFD),the condylar angle(CA),and the intercondylar angle(IA)of the ClassⅡ~2 group were smaller than those of ClassⅡ~1 and ClassⅠgroups(P<0.05).(2)The morphological measurements showed no significant difference between the two sides except for the CA of the ClassⅡ~2 group(P<0.05).(3)The morphological parameters showed no significant difference between different genders except for the GFW/GFD of the ClassⅡ~2 group(female>male,P<0.05).(4)The intergroup differences of female patients were shown in AEI-BFL,AEI-TRL and GFW/GFD(P<0.05).In addition,there were also statistical differences in GFW,CA and IA in female patients among different groups(P<0.05).Conclusion The morphology of the glenoid fossa reveals differences in the skeletal ClassⅠpatients with ClassⅠand ClassⅡmalocclusions,though the shape of the glenoid fossa was symmetrical on both sides.Gender has less influence on the glenoid fossa morphology of skeletal ClassⅠpatients with the same type of dental malocclusions.However,the differences in morphological parameters of TMJ in female individuals with different malocclusion are more significant than those in males.
