IVIM-DWI与DCE-MRI检查大鼠强直性脊柱炎骶髂早期改变 被引量:7

IVIM-DWI and DCE-MRI for early detection of sacroiliac changes in rat model of ankylosing spondylitis

摘要 [目的]使用磁共振IVIM-DWI及DCE技术扫描早期强直性脊柱炎(AS)大鼠模型,并与病理结果对照,评价磁共振诊断早期AS的应用价值。[方法]用牛蛋白聚糖联合完全/不完全弗式佐剂诱导20只Wistar雄性大鼠为实验组,20只健康同种属大鼠作为对照组。分别在诱导后第7、12、17、22周测量骶髂区IVIM-DWI及DCE-MRI序列参数,并取骶髂关节行组织病理检查。采用ROC曲线分析评价各参数的诊断效能。[结果]牛蛋白聚糖诱导后第7、12周两组间所有MRI参数均无统计学意义,病理未见异常。诱导后第17周,实验组Fenh%、Senh%和F值均显著高于对照组,两组间差别有统计学意义(P<0.05);实验组Dslow、Dfast及F值亦均高于对照组,但两组间差异无统计学意义(P>0.05);22周时,实验组参数Fenh%、Senh%、Dslow、Dfast及F值均显著高于对照组,差异具有统计学意义(P<0.05)。第17周实验组骶髂区滑膜区可见炎细胞浸润,第22周实验组骶髂区滑膜内出现血管翳,软骨表面渗出。ROC曲线分析表明:各参数中F值的曲线下面积最大(0.82),Senh%(s-1)具有的敏感性最高(71.4%),而F值的特异度最好(95.0%)。[结论]磁共振IVIM-DWI及DCE-MRI序列可敏感地监测强直性脊柱炎骶髂关节炎症早期局部微环境的改变,可为临床早期诊断AS提供可靠的影像学证据。 [Objective]To evaluate the significance of IVIM-DWI and DCE-MRI for detecting early sacroiliac changes in a rat model of ankylosing spondylitis(AS)by comparing with pathological consequences.[Methods]Twenty Wistar male rats in the AS group received bovine proteoglycan combined with complete/incomplete Freund’s adjuvant intraperiumally to create AS model,whereas other 20 male rats which received Freund’s adjuvant only without bovine proteoglycan were set as the control group.After IVIM-DWI and DCE-MRI examination,the parameters were compared on sacroiliac regions between the two groups at 7 th,12 th,17 th,and 22 thweeks after the last injection.In addition,the histopathological observation of sacroiliac joint was also conducted.The prediction of diagnostic efficiency of these parameters was compared by drawing receiver operating characteristic(ROC)curve.[Results]There was no significant difference in Fenh%,Senh%,Dslow,Dfast,and F value between the AS and control groups at the 7 and 12 weeks,associated with no positive findings in histopathological examination of sacroiliac joint.However,the Fenh%,Senh%and F value in the AS group proved significantly greater than those in the control group(P<0.05),regardless of no statistically significant differences in Dslow and Dfast between them at 17 weeks after injection(P>0.05),furthermore,all the aforesaid parameters became significantly greater in the AS group than the control group at22 weeks after injection(P<0.05).Correspondingly,histopathologic observation revealed inflammatory cell infiltration at 17 weeks and pannus forming of synovium on the surface of cartilage at the 22 weeks in AS groups.In term of ROC curve,the F value had the largest areas under the curve(0.82),Senh%(s-1)had the highest sensitivity(71.4%)and the F value had the highest specificity(95.0%)among these parameters for detection of the early change of sacroiliac joint of AS.[Conclusion]IVIM-DWI and DCE-MRI can be used as the sensitive imaging methods to detect the early active sacroiliitis in AS,which may be an additional tool for early and accurate diagnosis of AS.
