摘要
目的观察糖尿病视网膜病变(DR)患者血清CTRP3、Chemerin水平,并分析其与患者糖脂代谢的相关性。方法选取浙江省常山县人民医院2017年10月-2019年10月就诊的258例T2DM患者,将单纯T2DM患者89例作为T2DM组,合并2型糖尿病视网膜病变者依据DR病情分期不同分为增生型DR组(78例)和非增生型DR组(91例),选取同期体检健康者60例为对照组。检测四组血脂指标:甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C);血糖指标:空腹血糖(FPG)、餐后2h血糖(2h PG)、糖化血红蛋白(HbA1c),以及CTRP3、Chemerin水平,比较分析其差异,并分析CTRP3、Chemerin与糖脂代谢水平的相关性。结果增生型DR组CTRP3水平低于T2DM组、非增生型DR组以及对照组;Chemerin水平高于T2DM组、非增生型DR组以及对照组;非增生型DR组CTRP3水平低于T2DM组以及对照组;Chemerin水平高于T2DM组以及对照组;差异均有统计学意义(P<0.05)。增生型DR组FPG、2h PG、HbA1c水平高于T2DM组、非增生型DR组以及对照组;非增生型DR组FPG、2h PG、HbA1c水平高于T2DM组以及对照组;差异均有统计学意义(P<0.05)。增生型DR组TG、TC、LDL-C以及HDL-C水平高于T2DM组、非增生型DR组以及对照组;非增生型DR组TG、TC、LDL-C以及HDL-C水平高于T2DM组以及对照组;差异均有统计学意义(P<0.05)。DR患者血清CTRP3与糖脂代谢指标呈负相关(r值分别为-0.496、-0.135、-0.196、-0.471、-0.273、-0.229、-0.227,P<0.05);而Chemerin与其呈正相关(r值分别为0.307、0.136、0.186、0.337、0.267、0.080、0.125,P<0.05)。结论CTRP3、Chemerin与DR患者的糖脂代谢异常有一定相关性,临床可利用CTRP3、Chemerin评估糖尿病患者的视网膜病变。
Objective Serum CTRP3 and chemerin levels in patients with diabetic retinopathy(DR),and to analyze their correlation with patients with glucose and lipid metabolism.Methods 258 patients with T2DM who were treated at Changshan County People’s Hospital in Zhejiang Province from October 2017 to October 2019 were selected as the research object,and 89 patients with pure T2DM were selected as the T2DM group.Among them,DR patients were divided into hyperplasia according to different stages of DR.In the type DR group(78 cases)and the non-proliferative DR group(91 cases),60 healthy patients during the same period of physical examination were selected as the control group.Detect four groups of blood lipid indicators:triglycerides(TG),total cholesterol(TC),low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C);blood glucose indicators:fasting blood glucose(FPG),postprandial 2 h blood glucose(2 h PG),glycosylated hemoglobin(HbA;),CTRP3,chemerin levels,compare and analyze their differences,and analyze the correlation between CTRP3,chemerin and the level of glucose and lipid metabolism.Results The CTRP3 level of the proliferative DR group was lower than that of the T2DM group,non-proliferative DR group and the control group;Chemerin level was higher than that of the T2DM group,non-proliferative DR group and the control group;the CTRP3 level of the non-proliferative DR group was lower than that of the T2DM group and Control group;Chemerin level was higher than T2DM group and control group;the differences were statistically significant(P<0.05).The levels of FPG,2 h PG and HbA;in the proliferative DR group were higher than those in the T2DM group,non-proliferative DR group and the control group;the levels of FPG,2 h PG and HbA;in the non-proliferative DR group were higher than those in the T2DM group and the control group;the differences were statistically significant(P<0.05).The levels of TG,TC,LDL-C and HDL-C in the proliferative DR group were higher than those in the T2DM group,non-proliferative DR group and the control group;the levels of TG、TC、LDL-C and HDL-C in the non-proliferative DR group were higher than those in T2DM Group and control group;the differences were statistically significant(P<0.05).Serum CTRP3 of DR patients was negatively correlated with glucose and lipid metabolism indexes(r=-0.496、-0.135、-0.196、-0.471、-0.273、-0.229、-0.227,P<0.05);Chemerin was positively correlated with it(r=0.307、0.136、0.186、0.337、0.267、0.080、0.125,P<0.05)Conclusion CTRP3 and Chemerin are related to abnormal glucose and lipid metabolism in patients with DR.CTRP3 and Chemerin can be detected clinically to promptly treat the retinopathy of diabetic patients.evaluation of.
