白喉乌头改善类风湿关节炎大鼠滑膜增生的作用机制

摘要 目的本研究建立类风湿性关节炎(rheumatoid arthritis,RA)大鼠模型,探讨白喉乌头是否具有改善类风湿关节炎大鼠滑膜增生的作用及机制,为RA的治疗提供新策略。方法选取SPF级Wistar大鼠(新疆大学医学院动物实验中心),7~8周龄雌性大鼠,分5组,每组8只,共40只。其中空白组采取生理盐水皮内注射进行对照;其余组大鼠皮内注射牛Ⅱ型胶原乳剂以建造RA模型并鉴定,给予生理盐水1mL灌胃,1次/d,连续4周;低剂量组开始灌胃给药,白喉乌头22.5mg/kg溶于10mL生理盐水中配制成药液,每日给予1mL药液,连续给药4周;高剂量组灌胃给药,白喉乌头45mg/kg溶于10mL生理盐水中配制成药液,每日给予1mL药液,连续给药4周;阳性对照组:雷公藤1mg/kg溶于10mL生理盐水中配制成药液,每日给予1mL药液,连续给药4周。小鼠关节滑膜组织形态观察采用HE染色法;TNF-α、IL-6、IL-1β、IL-10、IL-12水平测定采用ELISA法。结果HE染色结果显示,空白对照组滑膜平整光滑;模型组滑膜组织增生显著且伴有水肿;低剂量组纤维增生少,高剂量组则无增生组织变化,水肿程度轻;与模型组相比,空白对照组、低剂量组、高剂量组和阳性对照组TNF-ɑ、IL-6和IL-1β表达量均有显著差异(P<0.05),且低、高剂量组之间TNF-ɑ表达量差异也较为显著(P<0.05),其他各组无显著差异(P>0.05);各组IL-10表达量无差异(P<0.05);IL-12表达量只在模型组和高剂量组间有明显差异(P<0.05)。结论白喉乌头能显著降低RA大鼠滑膜组织中相关炎症因子TNF-α、IL-6、IL-1β、IL-10、IL-12的水平,从而减轻滑膜组织炎症反应,减轻关节滑膜的纤维组织增生,防止关节的进一步破坏。 Objective The study established a rat model of rheumatoid arthritis(RA)and investigate whether aconitum diphtheriae could have effect and mechanism of improving synovial hyperplasia in RA rats,to provide new strategy for RA treatment.Methods The paper chose SPF grade Wistar rats(Animal Experimental Center,Xinjiang University Medicine School)and 7-8 week old female rats,and divided them into 5 groups,with 8 rats in each group,totally 40 rats.Control group was treated with intradermal injection of normal saline;in other groups,rats were injected intradermal injection with bovine type II collagen emulsion to build RA models and identify them,with gavage of 1mL of normal saline once a day for 4 weeks.Low-dose group was treated with 1 ml/dose of diphtheria aconite solution per day,with content of 22.5 mg/kg/d for 4 weeks;high-dose group was treated with solution of diphtheria aconite 1mL/time/day,with diphtheria aconite content of 45mg/kg/d,for 4 weeks of administration;positive control group was treated with 1 ml/dose of Tripterygium wilfordii solution per day,with Tripterygium wilfordii content of 1 mg/kg/day,for 4 weeks.Morphology of mouse joint synovial tissue was observed HE staining method;levels of TNF-α,IL-6,IL-1β,of IL-10 and IL-12 were measured with ELISA method.Results HE staining results showed,synovial membrane of blank control group was flat and smooth;synovial tissue in model group showed significant proliferation and accompanied by edema.Low-dose group showed less fibrous hyperplasia,while high-dose group showed no changes in proliferative tissue with mild edema.Compared with model group,TNF-ɑ,IL-6 and IL-1βexpression levels of blank control group,low-dose group,high-dose group,and positive control group showed significant difference(P<0.05),and TNF-ɑexpression significant showed difference between low-dose group and high-dose group(P<0.05),there was no significant difference in other groups(P>0.05).There was no difference in IL-10 expression between groups(P<0.05).Expression level of IL-12 showed significant difference only between model group and high-dose group(P<0.05).Conclusion Aconitum leucostomum can reduce inflammatory factor TNF-α,IL-6,IL-1β,IL-10 and IL-12 levels in synovial tissue of RA rats significantly,reduce inflammatory response of synovial tissue,alleviate proliferation of fibrous tissue in joint synovium,and prevent further damage to joints.
