摘要
目的分析在肺炎性肺癌患者诊断中应用CT检查的临床价值。方法从2018年12月至2021年1月择取80例肺炎性肺癌患者,所选患者均实施CT检查,对检查结果进行分析。结果从病理结果分析,80例肺炎性肺癌患者中,8例经手术病理检查确诊,20例经痰脱落细胞学检查确诊,26例经纤维支气管镜检查确诊,26例经胸部穿刺活检确诊。从病变位置分析,14例位于右肺上叶,4例位于右肺中叶,22例位于右肺下叶,14例位于左肺上叶,14例位于左肺下叶,12例累及双肺;72例肺外周部病变,8例中央部及外周部病变;44例为段性分布,36例叶性分布。从CT征象分析,80例肺炎性肺癌患者均发生实变,叶间裂膨隆/胸膜牵拉增厚76例,支气管充气征76例,小结节70例,枯枝征66例,纵膈淋巴结大/肺门增大60例,磨玻璃密度60例,空泡征60例,胸腔积液52例,网格征42例,蜂窝征24例,骨质破坏20例。结论对于肺炎性肺癌患者而言,通过CT检查可以观察病变位置,并结合CT征象,为临床诊断提供可靠依据。
Objective To analyze the clinical value of CT in the diagnosis of patients with pneumonic lung cancer.Methods Eighty patients with pneumonic lung cancer were selected from December 2018 to January 2021.All the selected patients underwent CT examination,and the examination results were analyzed.Results Among the 80 patients with pneumonic lung cancer,8 cases were confirmed by surgical pathology,20 cases by sputum exudation cytology,26 cases by fiberoptic bronchoscopy and 26 cases by chest puncture biopsy.From the analysis of lesion location,the lesion was located at the upper lobe of right lung in 14 cases,at the middle lobe of right lung in 4 cases,at the lower lobe of right lung in 22 cases,at the upper lobe of left lung in 14 cases,at the lower lobe of left lung in 14 cases,and at both lungs in 12 cases.There are 72 cases of peripheral lung lesions,8 cases of central and peripheral lung lesions.44 cases showed segmental distribution and 36 showed lobular distribution.From the analysis of CT signs,consolidation occurred in 80 patients with pneumonia lung cancer,including 76 cases of interlobar fissure swelling/pleural traction thickening,76 cases of bronchial inflation sign,70 cases of small nodules,66 cases of dead branch sign,60 cases of large mediastinal lymph nodes/hilar enlargement,60 cases of ground glass density,60 cases of vacuole sign,52 cases of pleural effusion,42 cases of grid sign,24 cases of honeycomb sign and 20 cases of bone destruction.Conclusion For patients with pneumonic lung cancer,CT examination can be used to observe the lesion location and provide a reliable basis for clinical diagnosis in combination with CT signs.
