摘要
目的本研究对拟通过经典方剂黄芪建中汤加减合并使用常规西药治疗脾胃虚寒型胃溃疡,观察其临床疗效以及对幽门螺旋杆菌的影响,探讨中西药治疗脾胃虚寒型胃溃疡的协同作用。方法采用随机对照试验的研究方法,在武威市中医医院消化科门诊收集符合诊断标准的脾胃虚寒型胃溃疡合并幽门螺旋杆菌阳性病例约90例患者,记录患者的一般资料、各种临床症状。随机分为对照组45例(采用常规西药治疗)与治疗组45例(常规西药治疗+黄芪建中汤加味治疗),两组均以1个月为1个疗程,2个疗程后对比疗效,复查胃镜或上消化道钡餐,复查14C呼气试验。观察两组的临床疗效以及对幽门螺旋杆菌的影响,探讨中西药治疗脾胃虚寒型胃溃疡的协同作用。结果在治疗后,治疗组显效、有效和无效分别为27例、16例和2例,总有效率为95.6%;对照组显效、有效和无效分别为18例、15例和12例,总有效率为73.3%,治疗组总有效率显著高于对照组(P<0.05)。结论临床用经典方剂黄芪建中汤加味合并使用常规西药治疗脾胃虚寒性胃溃疡,以及对幽门螺杆菌的治疗,其疗效明显高于单纯使用常规西药治疗,发挥中医的协同治疗作用,能够显著提升此类胃溃疡的治疗效率。
Objective This study is to observe the clinical effect of Huangqi Jianzhong decoction combined with conventional western medicine in treating gastric ulcer of spleen and stomach deficiency cold type,and its effect on Helicobacter pylori,to explore the synergistic effect of traditional Chinese and western medicine in treating gastric ulcer of spleen-stomach deficiency cold type.Methods In this study,about 90 patients with gastric ulcer of spleen-stomach deficiency cold type combined with Helicobacter pylori positive cases were collected from the outpatient department of gastroenterology of Wuwei Hospital of traditional Chinese medicine,the General Information and clinical symptoms of the patients were recorded.Randomly divided into the control group(45 cases,treated with conventional western medicine)and the treatment group(45 cases,treated with conventional western medicine+Modified Huangqi Jianzhong Decoction).The two groups were treated with one month as a course of treatment,and the curative effect was compared after two courses,gastroscopy or Barium meal of upper digestive tract were reexamined and 14C breath test was reexamined.Finally,through the comparison of monitoring indicators,to observe the clinical effect of the two groups and the influence on Helicobacter pylori to explore the synergistic effect of traditional Chinese and western medicine treatment of spleen and stomach cold gastric ulcer.Results After treatment,27 cases were effective,16 cases were effective and 2 cases were ineffective in the treatment group,with a total effective rate of 95.6%.Eighteen cases were effective,15 cases were effective and 12 cases were ineffective in the control group,with a total effective rate of 73.3%.The total effective rate of the treatment group was significantly higher than that of the control group(P<0.05).Conclusion The clinical efficacy of Huangqi Jianzhong decoction combined with conventional western medicine in the treatment of cold gastric ulcer of spleen and stomach deficiency,as well as the treatment of Helicobacter pylori,was significantly higher than that of conventional western medicine alone,and the synergistic therapeutic effect of traditional Chinese medicine can significantly improve the therapeutic efficiency of this kind of gastric ulcer.
