摘要
目的分析糖尿病肾病患者自身免疫指标及微炎症指标间的相关性与肾功能关系。方法选取2015年6月~2018年6月金华市人民医院收治的糖尿病肾病患者70例及单纯糖尿病患者30例作为对照。比较两组患者的肾功能指标(eGFR、Cre、BUN、CysC、NGAL)、自身免疫指标(Anti-GBM、ACA-IgA)、微炎症指标(hs-CRP)的差异。根据计算eGFR值,将糖尿病肾病患者分为预后良好组与预后不良组,比较不同预后患者上述指标差异。结果糖尿病肾病组eGFR值(84.64±20.33)低于单纯糖尿病组(152.58±31.24);糖尿病肾病组Cre、BUN、CysC、NGAL、Anti-GBM、ACA-IgA及hs-CRP水平高于单纯糖尿病组,差异均有统计学意义(t=18.384,11.561,8.633,20.549,6.926,6.238,12.039;P<0.05)。与预后不良组比较,预后良好组Cre、BUN、CysC、NGAL、Anti-GBM、ACA-IgA及hs-CRP水平显著降低,差异有统计学意义(t=24.639,14.692,9.048,31.069,7.137,6.466,16.528;P<0.05)。hs-CRP、Anti-GBM及ACA-IgA与eGFR呈负相关(r=-0.312,-0.366,-0.351;P<0.05);hs-CRP、Anti-GBM及ACA-IgA与Cre、BUN、CysC、NGAL均呈正相关(P<0.05)。结论自身免疫及微炎症指标在糖尿病肾病患者中均异常表达,可能参与了糖尿病肾病的疫情发展。
Objective To discuss the association between autoimmune and microinflammatory markers and its relationship with renal function in patients with diabetic nephropathy.Methods From June 2015 to June 2018,70 patients with diabetic nephropathy and 30 patients with diabetes mellitus alone were enrolled into our study observation group and control group.Renal function index(eGFR,Cre,BUN,CysC,NGAL),autoimmune index(Anti-GBM,ACA-IgA)and microinflammatory maker(hs-CRP)were compared between the two groups.According to the eGFR value,patients with diabetic nephropathy were divided into favorable prognosis group and poor prognosis group,and the above parameters were also compared between patients in the two groups.Results The eGFR value of patients with diabetic nephropathy(84.64±20.33)was significantly lower than that of those with diabetes mellitus alone(152.58±31.24),while the levels of Cre,BUN,CysC,NGAL,Anti-GBM,ACA-IgA and hs-CRP were significantly higher(t=18.384,11.561,8.633,20.549,6.926,6.238,12.039;P<0.05),especially in those with poor prognosis(t=24.639,14.692,9.048,31.069,7.137,6.466,16.528;P<0.05).Hs-CRP,Anti-GBM and ACA-IgA were negatively correlated with eGFR(r=-0.312,-0.366,-0.351;P<0.05),while were positively correlated with Cre,BUN,CysC,NGAL(P<0.05).Conclusions The abnormal expression of autoimmune and microinflammatory markers in patients with diabetic nephropathy may be responsible for the pathogenesis of diabetic nephropathy.
