摘要
目的对比多排螺旋CT和心血管造影对心肌桥-壁冠状动脉的诊断价值。方法回顾性分析2018年7月至2019年5月173例在李惠利医院东部院区影像科同时接受多排螺旋CT和心血管造影的心肌桥-壁冠状动脉患者的影像学资料,观察壁冠状动脉长度、埋藏深度及狭窄程度,分析多排螺旋CT和心血管造影对冠状动脉心肌桥的诊断价值。结果多排螺旋CT诊断壁冠状动脉平均长度为(7.73±2.28)mm,平均埋藏深度为(3.85±1.01)mm,平均狭窄程度为(37.18±10.13)%。心血管造影诊断壁冠状动脉平均长度为(4.93±1.12)mm,平均埋藏深度为(3.12±0.98)mm,平均狭窄程度为(52.18±13.38)%,差异有统计学意义(t=14.498、6.823、-11.756,P均<0.01)。壁冠状动脉狭窄程度和埋藏深度之间具有相关性,深埋型壁冠状动脉狭窄程度较表浅型壁冠状动脉狭窄程度更严重(t=12.626、11.702,P均<0.01)。多排螺旋CT和心血管造影对浅在型冠状动脉心肌桥和深埋型冠状动脉心肌桥的诊断上差异无统计学意义(P>0.05)。结论多排螺旋CT和心血管造影是诊断心肌桥-壁冠状动脉的有效检查方段,在临床应用中具有诊断的一致性。
Objective To compare the diagnostic efficacy of multi-slice spiral CT and angiocardiography on myocardial bridge-mural coronary artery.Methods From July 2018 to May 2019,the imaging data of 173 patients with myocardial bridge(MB)-mural coronary artery(MCA)confirmed by multi-slice spiral CT and cardiovascular angiography in our hospital were retrospectively analyzed.The length,burial depth and stenosis level of MCA were observed,and the diagnostic value of multi-slice spiral CT and cardiovascular angiography in MB-MCA were evaluated.Results The average length,average burial depth and average stenosis level of MCA were(7.73±2.28)mm,(3.85±1.01)mm and(37.18±10.13)%determined by multi-slice spiral CT,and were(4.93±1.12)mm,(3.12±0.98)mm and(52.18±13.38)%determined by angiocardiography,the differences were statistically significant(t=14.498,6.823,-11.756,P<0.05).There was a correlation between the stenosis level and burial depth of MCA,and the stenosis level of deeply buried MCA was higher than superficially buried MCA(t=12.626,11.702,P<0.05).There is no significant difference in the diagnosis of deep-type and superficial-type MB between multi-slice spiral CT and cardiovascular angiography(P>0.05).Conclusions Multi-slice spiral CT and cardiovascular angiography are both effective methods in the diagnosis of MB-MCA with high coincidence rate.
