摘要
介绍陆芝兰教授运用疏肝清热、软坚散结法治疗Graves眼病经验。陆芝兰教授认为Graves眼病病机为肝火旺盛、瘀血阻滞,治疗当以疏肝清热、软坚散结为主,方用自拟平突方,并根据患者临床症状进行适当加减,临床疗效显著。自拟平突方为陆芝兰教授临床20余年的经验方,其主要由生地黄、赤芍、浙贝母、穿山甲及三七粉等组成,经临床验证,该方效果显著、复发率低,且患者的经济负担小、依从性好。而目前西医对于Graves眼病的治疗主要依赖于糖皮质激素,虽然近年来新型免疫抑制剂成为研究热点,但其治疗毒副作用较大,患者病情易反复且依从性差,诸如此类因素导致该病治疗难度极大,患者生活质量也严重下降。由此,中医在治疗Graves眼病疗效及远期预后上高下立见。
This paper introduces Professor Lu Zhilan’s experience in the treatment of Graves ophthalmopathy by liver-soothing, heat-clearing, hard mass-softening and resolving therapy. Professor Lu Zhilan believes that Graves ophthalmopathy is caused by over abundent liver fire and blood stasis, which should be treated mainly by liver-soothing, heat-clearing, hard mass-softening and resolving therapy. Self-composed Pintu prescription with appropriate modification according to the clinical symptoms of patients, received significant clinical efficacy. Self-composed Pintu prescription is the fruit of more than 20 years’ clinical experience from Professor Lu Zhilan, and it is mainly composed of Shengdihuang(radix rehmanniae), Chishao(radix paeoniae rubra), Zhebeimu(thunberg fritillary bulb), Chuanshanjia(pangolin) and Sanqifen(panax notoginseng powder) etc.Clinical evaluation showed this prescription was of significant effect, low recurrence rate, affordability and good compliance. The current western solution on Graves ophthalmopathy mainly depends on glucocorticoids. Although new immunosuppressor becomes the research hot spot in recent years, it has disadvantages as side effects, recurrent tendency and poor compliance. These factors lead to unfavorable effect of treatment and serious decline of patient quality of life. Therefore, TCM is highly effective in treating Graves ophthalmopathy and improving long-term prognosis.
