SS18-SSX、SOX-2和PAX-7抗体在滑膜肉瘤病理诊断中的应用价值

The Value of SS18-SSX,SOX-2 and PAX-7 antibodies in the pathological diagnosis of synovial sarcoma

摘要 目的研究SS18-SSX、SOX-2及PAX-7抗体在滑膜肉瘤病理诊断中的作用。方法采用免疫组化检测11例滑膜肉瘤(经分子病理学检测证实)和32例软组织肿瘤中SS18-SSX、SOX-2及PAX-7表达。结果SS18-SSX在滑膜肉瘤中均呈弥漫性表达,而在非滑膜肉瘤中表达均为阴性,表达率有统计学差异(χ^(2)=43.0,P<0.01);SOX-2(χ^(2)=0.754)与PAX-7(χ^(2)=0.193)在非双相型滑膜肉瘤组和非滑膜肉瘤组中表达率的差异均无统计学意义(P>0.05),但SOX-2(χ^(2)=4.412)与PAX-7(χ^(2)=11.0)在双相型滑膜肉瘤组和非双相型滑膜肉瘤组中表达率的差异有统计学意义(P<0.05),两者在上皮样或腺样肿瘤细胞中呈弥漫性表达,而在梭形肿瘤细胞呈阴性或局灶弱表达。结论SS18-SSX抗体敏感度和特异度高,可用于大多数滑膜肉瘤的病理诊断;SOX-2与PAX-7具有特殊的表达模式,可辅助滑膜肉瘤组织学分类。 Objective To investigate the roles of SS18-SSX,SOX-2 and PAX-7 antibodies in the differential diagnosis of synovial sarcoma(SS).Methods The FFPE surgical tumor specimen from 11 genetically-confirmed SS and 32 morphological mimics were selected and the immunohistochemical staining was used to detect the SS18-SSX,SOX-2 and PAX-7 protein expression in the whole section of these specimen.Results SS18-SSX immunostaining was found diffusely positive in all SS,but negative in all morphological mimics(P<0.05).SOX-2 immunostaining was positive in 4 biphasic SS and 1 monophasic fibrous SS,whereas SOX-2 immunostaining was positive in 2 morphological mimics(P>0.05).PAX-7 immunostaining was positive in 5 biphasic SS and negative in 6 non-biphasic SS,whereas PAX-7 immunostaining was positive in 1 morphological mimics(P>0.05).Of note,PAX-7 expression was diffusely weak or moderate positive in the epithelial component of biphasic SS,but negative in majority of spindle tumor cells.Although SOX-2 immunostaining was found in both epithelial and spindle components of SS,its expression was more extensive and stronger in the epithelial and glandular tumor cells than spindle tumor components.Conclusion The high sensitivity and specificity of SS18-SSX fusion-specific antibody can be applied in the pathological diagnosis in majority of SS without the molecular pathological detection.The SOX-2 and PAX-7 immunostaining present a special pattern in the biphasic synovial sarcoma,and therefore they can be combined and used to classify the morphological variant in equivocal SS cases.
