糖化血红蛋白、血清总胆红素水平与糖尿病血管并发症发生的相关性 被引量:2

Correlation analysis of glycosylated hemoglobin, serum total bilirubin levels and the occurrence of diabetic vascular complications

摘要 目的探讨糖化血红蛋白(glycosylated hemoglobin,HbA1c)、血清总胆红素(total bilirubin,TBIL)水平与糖尿病血管并发症发生的相关性。方法选取2020年1至11月南京市雨花医院收治的100例糖尿病患者作为研究对象,根据有无血管并发症分为血管病变组(n=48)及无血管病变组(n=52),两组均行HbA1c、血清TBIL检测,分析上述两项指标与糖尿病血管并发症发生的关系。结果血管病变组性别、年龄、体质量指数(body mass index,BMI)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)与无血管病变组比较差异无统计学意义(P>0.05),血管病变组甘油三酯(triglycerides,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、HbA1c、血清TBIL水平均与无血管病变组差异有统计学意义(P<0.05);将有无血管并发症作为因变量,以病程、TBIL、HbA1c作为自变量纳入logistic回归方程中,结果显示病程、TBIL、HbA1c为糖尿病血管并发症的影响因素,差异有统计学意义(P<0.05),其中TBIL为保护因素,病程及HbA1c为危险因素;经Pearson相关性分析,TBIL与糖尿病血管并发症呈负相关(r=-0.734,P<0.05),HbA1c与糖尿病血管并发症呈正相关(r=0.871,P<0.05)。结论TBIL、HbA1c水平与糖尿病血管并发症发生具有密切关系,临床应重视糖尿病患者TBIL、HbA1c水平,并及时采取合理措施干预,积极预防糖尿病血管并发症发生。 Objective To explore the relationship between glycosylated hemoglobin(HbA1 c),serum total bilirubin(TBIL)levels and the occurrence of diabetic vascular complications.Methods Totally 100 diabetic patients admitted from January to November 2020 in Nanjing Yuhua Hospital were selected and divide into vascular disease group(n=48)and non-vascular disease group(n=52)according to whether they have vascular complications.Both groups were tested for HbA1 cand serum TBIL.The relationship between the above two indicators and the occurrence of diabetic vascular complications were analyzed.Results There were no statistically significant difference in gender,age,body mass index(BMI),high density lipoprotein cholesterol(HDL-C)between the vascular disease group and the non-vascular disease group(P>0.05).There were significant differences in triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),HbA1 c,and serum TBIL levels between the two groups(P<0.05).Taking the presence or absence of vascular complications as the dependent variable,and taking the course of disease,TBIL,and HbA1 cas independent variables,and they were included in the logistic regression equation.The results showed that the course of disease,TBIL,and HbA1 cwere the influencing factors of diabetic vascular complications(P<0.05).Among them,TBIL was a protective factor,disease course and HbA1 cwere risk factors.Pearson correlation analysis showed that TBIL was negatively correlated with diabetic vascular complications(r=-0.734,P<0.05),and HbA1 cwas positively correlated with diabetic vascular complications(r=0.871,P<0.05).Conclusion The levels of TBIL and HbA1 care closely related to the occurrence of diabetic vascular complications.Clinical parctitioners should pay attention to the levels of TBIL and HbA1 cin diabetic patients,and reasonable measures should be taken in time to prevent diabetic vascular complications.
