一起由肠道侵袭性大肠埃希菌引起的食物中毒实验室分析 被引量:3

Laboratory analysis on a case of food poisoning caused by invasive Escherichia coli

摘要 目的对食物中毒事件所采集的样品进行检测,查明本次中毒事件的致病因子,对后续治疗和预防控制工作提供实验室支持。方法运用实时荧光PCR和传统细菌分离鉴定两种方法对可疑食物中毒样品进行微生物检验。结果实时荧光PCR和血清凝集实验显示,此次事件的病原菌疑似志贺菌,生化鉴定为大肠埃希菌;菌株送上级疾控中心进行复检,经生化鉴定和致泻大肠埃希菌PCR确认试验,确定该事件病原菌为肠侵袭性大肠埃希菌。结论此次事件是一起由肠侵袭性大肠埃希菌引发的食物中毒事件,因志贺菌PCR试剂盒特异性不足、致泻大肠埃希菌和志贺菌存在血清交叉凝集等,导致本次检测没能及时得出正确结论。在食物中毒检验中,细菌学分离和鉴定是细菌检验定性的金标准,当其他检验方法结果与之矛盾时,应以生化反应结果为判定标准。 Objective To detect the samples collected from the cases of food poisoning to identify the pathogenic factors,and to provide laboratory support for the follow-up treatment and disease prevention.Methods The suspicious samples of food poisoning were detected by Real-time PCR and traditional isolation and identification methods.Results The results of both Real-time PCR and serum agglutination showed that the suspicious pathogenic bacteria was Shigella,and it was identified as Escherichia coli by biochemical identification.A invasive Escherichia coli(EIEC)was detected by superior CDC through biochemical identification and PCR confirmation test for lapactic Escherichia coli from the suspicious bacteria strains.Conclusion The incident was a case of food poisoning caused by invasive Escherichia coli(EIEC),and due to the lack of specificity of the Shigella PCR kit and the presence of serum cross-aggregation of lapactic Escherichia coli and Shigella,the test failed to get the correct conclusion in time.Bacteriological isolation and identification methods should be identified as the gold standard for food poisoning sample detection.When the results of other methods conflict with theirs,the results of biochemical reaction should be regarded as criterion.
