辅酶Q10联合维生素E对多囊卵巢综合征患者代谢和激素水平的影响 被引量:1

Effect of coenzyme Q10 combined with vitamin E on metabolism and hormone in patients with polycystic ovary syndrome

摘要 目的评价辅酶Q10单药或联合维生素E对多囊卵巢综合征(PCOS)患者葡萄糖稳态参数和生殖激素水平的影响。方法选取2016年10月至2018年9月在该院治疗的248例PCOS患者,根据随机数字表法随机分为辅酶Q10组、维生素E组、辅酶Q10联合维生素E治疗组(联合组)与安慰剂组(对照组),每组各62例。辅酶Q10组患者口服辅酶Q10片,剂量200 mg/d,3次/d。维生素E组患者服用维生素E片,联合组患者同时服用辅酶Q10片和维生素E片,对照组患者只服用安慰剂治疗。评价4组患者治疗后代谢和激素指标较治疗前的变化值及治疗前、治疗后各组患者代谢和激素指标的组间差异。采用线性回归分析评价各变量与游离雄激素指数(FAI)的相关性。结果辅酶Q10组与联合组患者治疗后空腹血糖(FPG)变化值较对照组显著提高(均P<0.05);辅酶Q10组与联合组患者稳态模式评估法测定胰岛素抵抗指数(HOMA-IR)变化值较对照组与维生素E组显著提高(均P<0.05);联合组与维生素E组治疗后总睾酮(TT)变化值均较对照组显著提高(均P<0.05);联合组治疗后性激素结合球蛋白(SHBG)变化值均较其他组显著提高(均P<0.05)。线性回归分析表明,FPG、胰岛素和HOMA-IR与FAI显著相关(P=0.032, P=0.028, P<0.001)。结论 PCOS患者应用辅酶Q10或与维生素E联合应用能改善血清FPG、胰岛素、HOMA-IR和TT水平,只有两种药物联用才会影响SHBG浓度。PCOS患者持续8周补充辅酶Q10可改善血清FPG,降低胰岛素和TT水平。 【Objective】 To evaluate the effects of coenzyme Q10 alone or in combination with vitamin E on reproductive hormone and glucose homeostasis in patients with polycystic ovary syndrome(PCOS).【Methods】A total of 248 patients with PCOS who were treated in our hospital from October 2016 to September 2018 were enrolled. Patients were randomly assigned to a coenzyme Q10 group, a vitamin E group, a coenzyme Q10 plus vitamin E treatment group(combined group) and a placebo group(control group) according to a random number table method, with 62 patients in each group. Patients in coenzyme Q10 group received oral CoQ10 tablets at a dose of 200 mg/d, 3 times/d. Patients in the vitamin E group took vitamin E tablets. The patients in the combined group received coenzyme Q10 tablets and vitamin E tablets. The control group received only placebo. The changes of metabolic and hormonal indexes after treatment in the four groups were compared with those before treatment, and the differences in metabolism and hormonal indexes between the groups before and after treatment were analyzed. Linear regression analysis was used to evaluate the correlation between each variable and free androgen index(FAI).【Results】The changes of fasting plasma glucose(FPG) in the coenzyme Q10 group and the combined group were significantly larger than those in the control group(both P<0.05).The changes of homeostatic model assessment for insulin resistance(HOMA-IR) in the coenzyme Q10 group and the combined group were significantly larger than those in the control group and the vitamin E group(both P<0.05). The total testosterone(TT)changes in the combined group and the vitamin E group were significantly larger than those in the control group(both P<0.05). The changes of sex hormone-binding globulin(SHBG) in the combined group were significantly larger than those in the other groups(all P<0.05). Linear regression analysis showed that FPG, insulin, and HOMA-IR were significantly associated with FAI(P=0.032, P=0.028, P<0.001).【Conclusion】Coenzyme Q10 or in combination with vitamin E used in PCOS patients can improve serum FPG,insulin, HOMA-IR and TT levels, only the combination of the two drugs will affect the SHBG concentration. Supplementation of coenzyme Q10 for 8 weeks in patients with PCOS improved serum FPG and decreased insulin and TT levels.
