摘要
[目的]系统评价黛力新联合选择性5-羟色胺再摄取抑制剂(SSRIs)治疗脑卒中后抑郁(PSD)的有效性和安全性.[方法]检索Cochrane library、PubMed、CNKI、VIP、ScienceDirect、Wan Fang、Web of Science等数据库,获得黛力新联合SSRIs治疗PSD的随机对照试验(RCTs)方面的文献,采用RevMan 5.3软件进行Meta分析.[结果]共纳入17篇文献,涉及1651例PSD患者.Meta分析结果显示,与SSRIs单独用药组比较黛力新联合SSRIs治疗PSD的总有效率显著升高,差异有统计学意义[RR=1.20,95%CI=(1.15,1.26),P<0.05];治疗1~6周时,黛力新联合SSRIs用药组汉密尔顿抑郁量表和美国国立卫生研究院卒中量表评分的降低幅度更为显著,差异均有统计学意义(P<0.05);两组不良反应发生率间差异无统计学意义(P>0.05).[结论]黛力新联合SSRIs治疗在改善PSD患者抑郁状态及神经功能缺损方面均优于SSRIs单药治疗,且具有相似的安全性.
OBJECTIVE To systematically evaluate the efficacy and safety of deanxit combined with selective serotonin reuptake inhibitors(SSRIs)in the treatment of post-stroke depression(PSD).METHODS Cochrane Library,PubMed,CNKI,VIP,ScienceDirect,Wan Fang and Web of Science databases were retrieved to obtain the literatures on randomized controlled trials(RCTs)of deanxit combined with SSRIs in the treatment of PSD.Meta-analysis was performed by using RevMan5.3 software.RESULTS A total of 17 literatures were included,involving 1651 patients with PSD.The results of Meta-analysis showed that compared with the SSRIs alone,the total effective rate of deanxit combined with SSRIs in the treatment of PSD was significantly increased,and the difference was statistically significant[RR=1.20,95%CI=(1.15,1.26),P<0.05].After 1 to 6 weeks of treatment,the decrease of Hamilton Depression Scale and the National Institutes of Health Stroke Scale scores in the deanxit combined with SSRIs group was more significant,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).CONCLUSION Deanxit combined with SSRIs is superior to SSRIs alone in improving depression and neurological function impairment in PSD patients,and has similar safety.
