自然周期监测排卵对150例不明原因不孕的临床意义

摘要 [背景]观察自然周期经阴道超声(TVU)监测卵泡对不明原因不孕(UI)病因研究及治疗的价值.[病例报告]选择UI患者150例(316个周期),均采取自然周期TVU监测卵泡方法观察卵泡生长速度、卵泡成熟后能否排出及排卵时卵泡大小,并结合激素水平及妊娠结局分析与不孕的相关性.监测排卵结果示,76.9%患者排卵正常,13.0%出现小卵泡排卵(OSF),7.0%发生卵泡黄素化不破裂(LUF).OSF组内膜薄,LH峰日雌二醇水平低于正常排卵组;OSF及LUF组排卵后1周孕酮水平均明显低于正常排卵组(P<0.01);OSF组临床妊娠率仅为4.9%,明显低于正常排卵组的14.8%(P<0.01).[讨论]自然周期TVU监测排卵对UI患者病因诊断及治疗具有意义. BACKGROUND To observe the value of natural cycle transvaginal ultrasound(TVU)monitoring follicle in the etiological study and treatment of unexplained infertility(UI).CASE REPORTS 150 UI patients were selected(316 cycles),the method of natural cycle TVU monitoring follicle was taken to observe the follicular growth rate,follicular discharge after maturation and follicular size during ovulation.The correlation with infertility was analyzed combining with hormone levels and pregnancy outcomes.Ovulation monitoring results showed that 76.9%of patients ovulated normal,13.0%of patients presented with ovulating with small follicles(OSF),and 7.0%of patients presented with luteinized unruptured follicle(LUF).OSF group was observed with thin endometrium,and the estradiol level was lower than that of normal ovulation group on LH peak day.The progesterone levels in OSF and LUF group were significantly lower than that in normal ovulation group on the day of 1 week after ovulation(P<0.01).Clinical pregnancy rate in OSF group was only 4.9%,significantly lower than the 14.8%in the normal ovulation group(P<0.01).DISCUSSION The monitoring of ovulation by natural cycle TVU was significant for the etiological diagnosis and treatment of UI patients.
