摘要
目的探讨2002、2007和2014年3个版本诊断标准对职业性噪声聋(ONID)诊断结论的影响。方法采用判断抽样方法,以1766名进行ONID诊断的劳动者为研究对象,收集其纯音听阈测试结果,分别按照GBZ 49—2002《职业性听力损伤诊断标准》(以下简称“GBZ 49—2002”)、GBZ 49—2007《职业性噪声聋诊断标准》(以下简称“GBZ 49—2007”)、GBZ 49—2014《职业性噪声聋的诊断》(以下简称“GBZ 49—2014”)进行ONID诊断,对诊断结果进行比较。结果分别与GBZ 49—2002和GBZ 49—2007比较,采用GBZ 49—2014对ONID的诊断率较高(57.9%vs 66.0%,44.8%vs 66.0%,P值均<0.01),对轻度ONID的诊断率较高(47.3%vs 54.6%,36.0%vs 54.6%,P值均<0.01)。在各个年龄组,采用GBZ 49—2014对ONID的诊断率亦分别较GBZ 49—2002和GBZ 49—2007有不同程度的升高(P值均<0.02)。结论与GBZ 49—2002和GBZ 49—2007比较,GBZ 49—2014提高了ONID的诊断率;其原因与GBZ 49—2014纳入权重为0.1的4000 Hz听阈值作为诊断听阈和使用新的年龄与性别修正方法有关。
Objective To explore the influence on the diagnosis of occupational noise-induced deafness(ONID)using three versions of diagnostic criteria in 2002,2007 and 2014.Methods A total of 1766 workers who asked for ONID diagnosis were selected as the research subjects using judgment sampling method.The results of pure tone audiometry were collected.The ONID was diagnosed using GBZ 49-2002 Diagnostic Criteria of Occupational Noise-induced Hearing Loss(hereinafter referred to as GBZ 49-2002),GBZ 49-2007 Diagnostic Criteria of Occupational Noise-induced Deafness(hereinafter referred to as GBZ 49-2007)and GBZ 49-2014 Diagnostic of Occupational Noise-induced Deafness(hereinafter referred to as GBZ 49-2014),and the diagnostic results were compared.Results Compared with GBZ 49-2002 and GBZ 49-2007,diagnosis with GBZ 49-2014 had a higher rate of ONID(57.9%vs 66.0%,44.8%vs 66.0%,both P<0.01),and had a higher rate of mild ONID(47.3%vs 54.6%,36.0%vs 54.6%,both P<0.01).The diagnostic rate for ONID using GBZ 49-2014 was higher than those using GBZ 49-2002 and GBZ 49-2007 in each age groups(all P<0.01).Conclusion GBZ 49-2014 improved the diagnostic rate of ONID compared with GBZ 49-2002 and GBZ 49-2007.The reason is related to the inclusion of 4000 Hz hearing threshold with a weight of 0.1 as the diagnostic hearing threshold and the use of a new age and gender correction method in GBZ 49-2014.
