摘要
目的探讨腔隙性脑梗死患者炎症因子水平与卒中后抑郁(PSD)的相关性,分析影响PSD的危险因素。方法回顾性分析安徽省合肥市第二人民医院在2018年4月至2019年4月收治的腔隙性脑梗死患者155例,根据发病后6个月内是否出现PSD分为PSD组(39例)和非PSD组(116例),分析两组患者临床资料,检测胱抑素-C(Cys-C)及超敏C反应蛋白(hsCRP)水平,比较两组患者MRI影像学资料;多因素回归分析引起PSD的危险因素。结果(1)155例患者中共出现39例PSD,发生率为25.16%;PSD组汉密尔顿抑郁量表(HAMD)评分比非PSD组高,组间比较差异具有统计学意义(t=15.618,P<0.05);(2)PSD组丘脑卒中、额叶卒中总负荷方面高于非PSD组(χ2=23.018,P<0.05);(3)炎症因子方面,PSD组Cys-C水平高于非PSD组(t=10.808,P<0.05);(4)多因素线性回归方程分析显示,丘脑卒中、额叶卒中(β=0.205,0.200,P<0.05)、Cys-C高表达(β=0.484,P<0.05)是导致PSD的独立危险因素。结论腔隙性脑梗死后通过早期发现丘脑卒中、额叶卒中、Cys-C高表达能够预测PSD的发生,做到早期干预可能会减少PSD的发生。
Objective To investigate the relationship between inflammatory factors and poststroke depression(PSD)in patients with lacunar infarction,and to analyze the risk factors affecting PSD.Methods 155 patients with lacunar infarction admitted to Hefei Second People’s Hospital from April 2018 to April 2019 were retrospectively analyzed.According to whether PSD was present within 6 months after onset,patients were divided into PSD group and non-PSD group.Clinical data of patients were tested for Cystatin-C(Cys-C)and hypersensitive C-reactive protein(hs-CRP)levels.MRI imaging data were compared between the two groups.Multivariate Linear regression was performed toanalyze the risk factors that cause PSD.Results(1)There were 39 cases of PSD in 155 patients,the incidence rate was 25.16%.Hamilton depression rating scale(HAMD)in PSD group was higher than that in nonPSD group,the difference was statistically significant(t=15.618,P<0.05).(2)The total load of thalamic stroke,frontal lobe stroke in PSD group was higher than that in non-PSD group,the difference was statistically significant(χ2=23.018,P<0.05).(3)In terms of inflammatory factors,the Cys-C level in the PSD group was higher than that in the non-PSD group,and the difference was statistically significant(t=10.808,P<0.05).(4)Multivariate linear regression equation analysis showed that thalamic stroke,frontal lobe stroke(β=0.205,0.200,P<0.05),Cys-C high expression(β=0.484,P<0.05)are independent risk factors for PSD.Conclusion PSD can be predicted by early detection of thalamic stroke,frontal lobe stroke and high expression of Cys-C after lacunar infarction.Early intervention may reduce the incidence of PSD.
