摘要
目的分析罗哌卡因髋关节囊周围神经阻滞应用浓度对髋关节置换术患者镇痛效果的影响。方法从2019年9月-2022年9月于我院行髋关节置换术治疗患者中抽取82例,按照随机数字表法将其均分为对照组和观察组,每组各41例。所有患者均在腰硬联合麻醉下行髋关节置换术治疗,在进行腰硬联合麻醉前30 min行罗哌卡因髋关节囊周围神经阻滞,对照组注射浓度0.2%,观察组注射浓度0.4%。对比分析两组患者术后疼痛程度、舒芬太尼累计用量以及不良反应发生情况。结果术后疼痛感程度评分比较:3、6 h差异无意义(P>0.05);术后12、24、48 h观察组评分均低于对照组,具有显著性差异(P<0.05)。整体分析结果:F_(组间)/F_(时间)/F_(交互)=23.985/67.362/1.321,P组间/P时间<0.001,P交互=0.270>0.05。术后舒芬太尼累计用量比较:观察组低于对照组,具有显著性差异(P<0.05)。整体分析结果:F_(组间)/F_(时间)/F_(交互)=2612.138/393.673/4.816,P组间/P时间/P交互<0.001。观察组不良反应发生率2.44%(1/41),显著低于对照组14.63%(6/41),差异存在统计学意义(P<0.05)。结论应用0.4%浓度的罗哌卡因行髋关节囊周围神经阻滞,可有效降低髋关节置换术后患者疼痛程度、舒芬太尼使用剂量以及不良反应发生率。
Objective:To analyze the effect of ropivacaine concentration on analgesic effect in patients undergoing hip arthroplasty.Methods:A total of 82 patients who underwent hip arthroplasty in our hospital from September 2019 to September 2022 were selected,and were divided into control group and observation group by random number table method,with 41 cases in each group.All patients were treated with hip arthroplasty under combined spinal and epidural anesthesia.Ropivacaine was performed for 30 min before combined spinal and epidural anesthesia,with the injection concentration of 0.2%in the control group and 0.4%in the observation group.The degree of postoperative pain,the cumulative dosage of sufentanil and the incidence of adverse reactions were compared and analyzed between the two groups.Results:There was no difference in postoperative pain score between 3 and 6 h(P>0.05).The scores of the observation group at 12,24 and 48 h after operation were lower than those of the control group,and the differences were significant(P<0.05).Overall analysis results:F_(group)/F_(time)/F_(interaction)=23.985/67.362/1.321,Pgroup/Ptime<0.001,Pinteraction=0.270>0.05.The cumulative dosage of sufentanil in the observation group was lower than that in the control group,and the difference was significant(P<0.05).Overall analysis results:F_(group)/F_(time)/F_(interaction)=2612.138/393.673/4.816,Pgroup/Ptime/P interaction<0.001.The incidence of adverse reactions in the observation group was 2.44%(1/41),which was significantly lower than 14.63%(6/41)in the control group,and the difference was statistically significant(P<0.05).Conclusion:Ropivacaine(0.4%)can effectively reduce the degree of pain,sufentanil dose and incidence of adverse reactions in patients after hip replacement.
