摘要
目的探讨远红外线热疗联合常规护理对糖尿病肾病血液透析患者瘘管血流量的影响。方法选择2017年4月至2019年8月在我院行血液透析的糖尿病肾病患者60例,根据抽签法将其分为两组,各30例。两组均行血液透析治疗,对照组给予常规护理干预,观察组在对照组基础上采用远红外线热疗,干预6个月后,比较两组瘘管血流量、并发症发生率及疼痛程度。结果干预6个月后,观察组瘘管血流量较干预前上升,且瘘管血流量较对照组高,差异具有可比性(P<0.05);对照组干预前、后干预6个月后瘘管血流量相比,无明显差异(P>0.05)。治疗期间,观察组并发症总发生率6.67%低于对照组26.67%,差异具有可比性(P<0.05)。干预后,两组VAS评分均较干预前降低,且观察组评分较对照组低,差异具有可比性(P<0.05)。结论远红外线热疗联合常规护理可提高糖尿病肾病血液透析患者瘘管血流量,降低疼痛程度及并发症发生率。
Objective This paper aims to investigate the effect of far infrared hyperthermia combined with conventional nursing on fistula blood flow in patients with diabetic nephropathy on hemodialysis.Methods 60 patients with diabetic nephropathy who underwent hemodialysis in our hospital from April 2017 to August 2019 were selected and divided into two groups according to the lottery method,with 30 cases in each group.Two groups were treated with hemodialysis,the control group was given routine nursing intervention,and the observation group was treated with far infrared hyperthermia on the basis of the control group.After 6 months of intervention,the fistula blood flow,complication rate and pain degree were compared between the two groups.Results After 6 months of intervention,the fistula blood flow in the observation group was higher than before intervention,and the fistula blood flow was higher than that in the control group,the difference was comparable(P<0.05);before intervention,after intervention and 6 months after intervention,compared with fistula blood flow,there was no significant difference(P>0.05);during the treatment period,the total incidence of complications in the observation group was 6.67%,lower than the 26.67%in the control group,and the difference was comparable(P<0.05);after intervention,the VAS scores of the two groups were lower than those before intervention,and the scores of the observation group were lower than those of the control group,and the differences were comparable(P<0.05).Conclusion Far infrared hyperthermia combined with conventional nursing care can increase the blood flow of the fistula in hemodialysis patients with diabetic nephropathy and reduce the degree of pain and the incidence of complications.
