摘要
目的探讨尿激酶肝素混合封管在带涤纶套双腔透析导管中的应用效果。方法选择本院2016年1月-2019年2月经带涤纶套双腔透析导管的66例实施血液透析患者作为研究对象,将其按照数字表法随机分为两组,即参照组与研究组,参照组采用肝素液封管(n=33例),研究组采用尿激酶肝素混合液封管(n=33例)。观察和比较两组患者的血液透析自发性出血、导管功能不良、导管更换等不良事件发生情况,检测两组患者导管血流量、静脉压。结果研究组血液透析1个月后的导管血流量、静脉压与参照组透析1个月后比较,差异无统计学意义(P>0.05);研究组血液透析6个月后的导管血流量低于透析1个月后,参照组血液透析6个月后的导管血流量低于透析1个月后,研究组透析6个月后的导管血流量高于参照组,差异具有统计学意义(P<0.05);两组血液透析6个月后的静脉压高于1个月后,研究组透析6个月后的静脉压低于参照组,差异具有统计学意义(P<0.05)。研究组导管不良事件总发生率低于参照组,差异具有统计学意义(P<0.05)。结论在带涤纶套双腔透析导管中应用尿激酶肝素混合封管的效果优于肝素液封管效果,可减少不良事件发生情况。
Objective This paper aims to explore the application effect of urokinase heparin mixed sealing in double chamber dialysis catheter with Dacron sleeve.Methods 66 hemodialysis patients with Dacron double lumen catheter in our hospital from January 2016 to February 2019 were randomly divided into two groups according to the method of digital table,namely,the reference group and the study group.The reference group was sealed with heparin solution(n=33 cases),and the study group was sealed with urokinase heparin mixture solution(n=33 cases).To observe and compare the occurrence of spontaneous haemorrhage,catheter dysfunction,catheter replacement and other adverse events in hemodialysis between the two groups.Results There was no significant difference in catheter blood flow and venous pressure between the study group and the reference group after hemodialysis for one month(P>0.05);the catheter blood flow of the study group after hemodialysis for six months was lower than that of the reference group after dialysis for one month,and the catheter blood flow of the reference group after hemodialysis for six months was lower than that of the reference group after dialysis for one month,and the catheter blood flow of the study group after dialysis for six months was higher than that of the reference group,and the difference was statistically significant(P>0.05)The difference was statistically significant(P<0.05);the venous pressure of the two groups after 6 months of hemodialysis was higher than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of catheter adverse events in the study group was lower than that in the reference group(P<0.05).Conclusion It is better to use urokinase and heparin to seal the catheter in Dacron double lumen dialysis catheter than heparin,which can reduce the incidence of adverse events.
