摘要
目的探究重症肺炎患者使用氨溴索、机械振动排痰联合纤维支气管镜对其肺功能的影响。方法研究对象选取重庆市人民医院2015—2020年收治的118例重症肺炎患者,使用随机数字表法将所有患者分为常规组及治疗组,每组59例。常规组采用常规治疗及纤维支气管镜治疗,治疗组加用氨溴索与机械振动排痰进行治疗。比较两组患者治疗有效率、症状改善时间、动脉血气指标及肺功能。结果治疗组治疗总有效率高于常规组(P<0.05);治疗组不良症状改善时间显著低于对照组(P<0.05);治疗前两组PaO2、SaO2、PaCO2比较差异无统计学意义(P>0.05),治疗后,治疗组PaO2、SaO2显著高于常规组,PaCO2显著低于常规组(P<0.05);治疗前,两组FVC、FEV1、FEV1/FVC水平比较差异无统计学意义(P>0.05),治疗后,治疗组FVC、FEV1、FEV1/FVC水平显著高于常规组(P<0.05)。结论氨溴素、机械振动排痰联合纤维支气管镜能有效改善重症肺炎患者血气指标,缩短患者症状缓解时间,提高治疗有效率及患者肺功能。
Objective To explore the effects of ambroxol,mechanical vibration sputum excretion combined with fiberoptic bronchoscopy on lung function in patients with severe pneumonia.Methods The study subjects selected 118 patients with severe pneumonia admitted to the hospital from 2015 to 2020.All patients were divided into routine group and treatment group using a random number table method,with 59 cases in each group.The conventional group was treated with conventional treatment and fiberoptic bronchoscopy,and the treatment group was treated with ambroxol and mechanical vibration expectoration.The treatment efficiency,symptom improvement time,arterial blood gas index and lung function were compared between the two groups.Results The total effective rate of treatment in the treatment group was higher than that in the conventional group(P<0.05);the improvement time of adverse symptoms in the treatment group was significantly lower than that in the control group(P<0.05);there was no significant difference in PaO2,SaO2,PaCO2 between the two groups before treatment(P>0.05).After treatment,PaO2 and SaO2 in the treatment group were significantly higher than those in the conventional group,and PaCO2 was significantly lower than the conventional group(P<0.05).Before treatment,there was no significant difference in the levels of FVC,FEV1,FEV1/FVC between the two groups(P>0.05).After treatment,the levels of FVC,FEV1,FEV1/FVC in the treatment group were significantly higher than those in the conventional group(P<0.05).Conclusion Ambroxol,mechanical vibration sputum expectoration combined with fiberoptic bronchoscopy can effectively improve blood gas indexes in patients with severe pneumonia,shorten the time of symptom relief,and improve the treatment efficiency and lung function of patients.
