摘要
目的观察冷空气冷却仪在Picoway皮秒激光治疗中的安全性和有效性。方法采用面部双侧对照试验,将37例光老化患者按照左右面部随机分为两组,A组在皮秒点阵激光治疗前30 min,将2%复方利多卡因乳膏涂在面部的一侧,B组在使用皮秒点阵激光治疗同时使用Zimmer冷却器设备4~5级进行冷空气冷却。皮秒点阵激光进行治疗时,1064 nm手具,6×6 mm光斑,能量1.7~2.1 J/cm^(2),532 nm手具,6×6 mm光斑,能量0.16~0.18 J/cm^(2)。于术前、术中及术后即刻记录皮肤温度,术中及术后疼痛感评估,术后1、7、30 d进行随访。Antera 3D皮肤成像检测仪对比术后1个月皮肤皱纹、毛孔、纹理、黑素、红斑指数等变化。结果术中及术后即刻B组温度低于A组(P<0.05);B组疼痛感低于A组(P<0.05)。皮秒激光治疗后1个月A组与B组黑素、皱纹、纹理、毛孔均较术前改善(P<0.05),但两组间差异无统计学意义。结论冷空气冷却仪与传统复方利多卡因软膏相比,其可以减轻皮秒激光治疗中患者的不适,但对疗效无明显影响。
Objective To observe the efficacy and safety of cold air cooling combined with Picoway in the treatment of facial photoaging.Method For 37 patients with fitzpatrick skin type Ⅱ to Ⅳ,split face study,different treatment strategies were applied.All patients were divided into 2 groups with 37 cases in each group.Group A was covered with compound lidocaine cream half an hour,then treated by Picoway;Group B was treated by cold air cooling and picoway at the same time.The cold air cooling device si Zimmer(LaserMed,Shelton,CT),used 4~5 leve to treat.When picoway treated,used the 1064 resolve handpiece,6×6 cm spot size,the energy was 1.7~2.1 J/cm^(2);and 532nm resolve handpiece,6×6 cm spot size,the energy was 0.16~0.18 J/cm^(2).The temperature of face was recorded before treatment,during treatment and imedddiately after treatment.Visual analogue scale(VAS)was uesd to evulated the pain level during and after the treatment.And efficacy was evaluated by the think view and Antera 3D image systems.Adverse reactions were recorded.Results The temperature of group B was lower than group A,no matter during or after treatment(P<0.05).The VAS score of group B is lower than group A(P<0.05),All patients were followed up for 1 months after treatment.37 patients had good treatment results,and Antera 3D showed that spots,wrinkles,texture,and pores in both group were improved(P<0.05).But no difference in both group.Conclusion Campred to compound lidocaine cream,cold air cooling device could reduce the pian in treatmend of Picoway,and no influence of laser result.
