摘要
目的目前临床尚未明确中心性浆液性脉络膜视网膜病变(central serous chorioretinopathy,CSC)有效安全的治疗方式,本研究探讨577nm阈下微脉冲矩阵激光治疗CSC临床效果。方法选取2017-12-03-2018-11-03郑州爱尔眼科医院门诊确诊的30例CSC患者30眼进行研究,对患者黄斑区盘状隆起处行577nm阈下微脉冲矩阵激光全覆盖光凝治疗。对比患者治疗后1周、1个月和3个月最佳矫正视力(best corrected visual acuity,BCVA)及黄斑中心凹视网膜厚度(central macular thickness,CMT)变化情况。结果所有患眼在随访过程中均未发生脉络膜及视网膜损伤等并发症。在治疗后3个月时,30眼中,24眼视网膜下液已完全吸收,6眼仍有视网膜下液积存,但较治疗前已有明显缓解趋势,同时对此6眼进行了二次微脉冲激光治疗。治疗前、治疗后1周、治疗后1个月和治疗后3个月患者BCVA分别为(0.35±0.10)、(0.34±0.09)、(0.07±0.06)和(0.07±0.06)LogMAR,差异有统计学意义,F=7.953,P<0.001,随时间变化呈升高趋势。治疗前、治疗后1周、治疗后1个月和治疗后3个月患者CMT分别为(381.47±16.47)、(337.97±13.66)、(226.97±5.33)和(226.43±5.29)μm,差异有统计学意义,F=12.724,P<0.001,随时间变化呈下降趋势。结论577nm阈下微脉冲矩阵激光治疗中心性浆液性脉络膜视网膜病变,可有效改善患者视力,降低CMT。
OBJECTIVE At present,the clinical and effective treatment of central serous chorioretinopathy(CSC)has not yet been clarified.This study explores the clinical effect of 577 nm subthreshold micropulse matrix laser treatment of CSC.METHODS A total of 30 eyes of 30 CSC patients diagnosed in the outpatient department of Zhengzhou Aier Eye Hospital were selected from December 3,2018 to March 11,2018.Coagulation treatment.The changes of best corrected visual acuity(BCVA)and central macular thickness(CMT)of the macular fovea at 1 week,1 month and 3 months after treatment were compared.RESULTS No complications such as choroid and retinal damage occurred in all affected eyes during follow-up.At 3 months after treatment,of the 30 eyes,24 eyes had fully absorbed subretinal fluid,and 6 eyes still had subretinal fluid accumulation,but there was a clear remission trend compared with before treatment.At the same time,6 eyes were given a second micropulse Laser treatment.The BCVA of patients before treatment,1 week after treatment,1 month after treatment and 3 months after treatment were(0.35±0.10),(0.34±0.09),(0.07±0.06)and(0.07±0.06)LogMAR,the difference was statistically significant,F=7.953,P<0.001,showing an increasing trend with time.The CMT of patients before treatment,1 week after treatment,1 month after treatment and 3 months after treatment were(381.47±16.47),(337.97±13.66),(226.97±5.33)and(226.43±5.29)μm,the difference was statistically significant,F=12.724,P<0.001,showing a downward trend with time.CONCLUSION 577 nm subthreshold micro-pulse matrix laser treatment of central serous chorioretinopathy could effectively improve patients’vision and reduce CMT.
