摘要
目的2型糖尿病发病隐匿,并发症发病率高,严重威胁人类健康。本研究分析2型糖尿病患者慢性并发症的危险因素,为临床防治提供参考。方法选取2011-01-01-2013-12-31丹东市第一医院住院的2型糖尿病患者为研究对象。选择其中发生并发症的816例患者为病例组(并发症组),123例无并发症的患者为对照组(非并发症组)。对其临床及实验室资料进行统计,以二分类Logistic回归法分析2型糖尿病患者慢性并发症的危险因素。结果单因素分析结果显示,年龄(t=11.153,P<0.001)、性别(χ2=10.813,P=0.001)、病程(t=7.131,P<0.001)、血压(收缩压t=8.390,P<0.001;舒张压t=2.020,P=0.044)、体质量指数(t=3.149,P=0.002)、三酰甘油(t=-2.324,P=0.020)及尿微量白蛋白/肌酐(t=6.533,P<0.001)与2型糖尿病慢性并发症的发生存在相关性。多因素分析结果显示,年龄(OR=1.109,95%CI为1.079~1.139,P<0.001)、病程(OR=1.091,95%CI为1.043~1.142,P<0.001)、收缩压(OR=1.027,95%CI为1.013~1.040,P<0.001)、体质量指数(OR=1.090,95%CI为1.013~1.172,P=0.022)和尿微量白蛋白/肌酐(OR=1.015,95%CI为1.007~1.024,P<0.001)是2型糖尿病慢性并发症发生的危险因素。结论年龄大、病程长、收缩压高、体质量指数大及尿微量白蛋白/肌酐水平高为慢性并发症发生的危险因素。
OBJECTIVE Type 2 diabetes is hidden and has a high incidence rate of complications,which seriously threatens human health.This study analyzed the risk factors of chronic complications in patients with type 2 diabetes mellitus to provide reference for clinical prevention and treatment.METHODS Patients with type 2 diabetes mellitus admitted to the First Hospital of Dandong City from January 1,2011 to December 31,2013 were selected as the study objects.A total of 816 patients with complications were selected as the case group(complications group)and 123 patients without complications was regarded as the control group(non complications group).The clinical and laboratory data were statistically analyzed,and the risk factors of chronic complications in type 2 diabetes mellitus patients were analyzed by binary Logistic regression.RESULTS The results of univariate analysis showed that age(t=11.153,P<0.001),gender(χ~2=10.813,P=0.001),course of disease(t=7.131,P<0.001),blood pressure(systolic pressure:t=8.390,P<0.001.diastolic pressure:t=2.020,P=0.044),body mass index(t=3.149,P=0.002),triacylglycerol(t=-2.324,P=0.020),and urinary microalbumin/creatinine(t=6.533,P<0.001)were correlated with the occurrence of chronic complications of type 2 diabetes.The results of multivariate analysis showed that age(OR=1.109,95%CI:1.079-1.139,P<0.001),Disease course(OR=1.091,95%CI:1.043-1.142,P<0.001),systolic blood pressure(OR=1.027,95%CI:1.013-1.040,P<0.001),body mass index(OR=1.090,95%CI:1.013-1.172,P=0.022),and urinary microalbumin/creatinine(OR=1.015,95%CI:1.007-1.024,P<0.001)were risk factors for the occurrence of chronic complications of type 2 diabetes.CONCLUSION The risk factors of chronic complications are old age,long course of disease,high systolic blood pressure,high body mass index and high level of urinary microalbumin/creatinine.
