摘要
目的产后缺乳指产妇哺乳期内乳汁甚少或者全无,影响母乳喂养和婴儿健康。本研究评价增乳膏对产后缺乳的疗效,为临床提供循证参考。方法检索知网、维普、万方、PubMed和Cochrane library数据库。检索时限为建库至2019-09-30,搜集妇女产后使用增乳膏的随机对照试验。由2名研究员按纳入排除标准独立筛选文献、提取资料和评价纳入文献质量。采用Revman 5.3对有效率、配方奶添加量和乳房胀度评分进行Meta分析。其他指标采用描述性评价。结果最终纳入文献5篇,共计1014例产妇。Meta分析结果显示,使用增乳膏与对照组相比,可提高治疗有效率(RD=0.05,95%CI为0.01~0.09,P=0.01)、减少配方奶添加量(随访第3~5天MD=-23.24,95%CI为-39.70^-6.79,P=0.006;随访第7天MD=-38.24,95%CI为-56.84^-19.63,P<0.001)以及提高乳房胀度评分(随访第7天MD=0.15,95%CI为0.08~0.22,P<0.001)。未见严重不良事件发生。结论增乳膏能有效治疗产后缺乳症。
OBJECTIVE Postpartum hypogalactia refers to the lactation period of puerpera with little or no milk,which affects breastfeeding and infant health.This study was to evaluate the efficacy of Zengru Gao on postpartum hypogalactia,and to provide evidence-based reference for clinical practice.METHODS The databases of CNKI,VIP,Wanfang,PubMed and Cochrane library were searched.Randomized controlled trials(RCTs)of postpartum women using Zengru Gao were collected from the database.The search period was from the establishment of the database to September 30,2019.Two researchers independently screened the literatures,according to the inclusion and exclusion criteria,extracted the materials,and evaluated the quality of the included literatures.Revman5.3 was used to conduct a Meta-analysis of the effective rate,the amount of formula milk added and the breast swelling score.The remaining indicators were descriptively evaluated.RESULTS A total of 1014 postpartum women were included in 5 RCTs.Meta analysis results showed that compared with the control group,using Zengru Gao could improve the effective rate of treatment(RD=0.05,95%CI was 0.01-0.09,P=0.01),reduce the amount of formula milk added(3-5 days of follow-up MD=-23.24,95%CI was-39.70--6.79,P=0.006;7 days of follow-up MD=-38.24,95%CI was-56.84--19.63,P<0.001),improve the breast swelling score(7 days of follow-up MD=0.15,95%CI was0.08-0.22,P<0.001).No serious adverse events reported.CONCLUSION Current evidence showed that Zengru Gao was effective in the treatment of postpartum breast milk deficiency.
