摘要
目的观察新益气养阴方联合化疗对妇科恶性肿瘤术后患者的疗效。方法将64例妇科恶性肿瘤(宫颈癌、子宫内膜癌、卵巢癌)Ⅰ~Ⅲ期患者随机分为试验组32例和对照组32例。两组化疗方案均为紫杉醇+铂类静脉给药,治疗6个疗程,试验组在化疗期内口服新益气养阴方。采用Karnofsky表及欧洲癌症研究机构与治疗组织(EORTC)的QLQ-C30评分表评价两组化疗期生存质量。同时观察两组的中医证候积分变化情况、血清CA125值及复发情况。结果治疗后,试验组较对照组气阴两虚证和血瘀证积分降低(P<0.05)。治疗后,试验组在躯体功能、情绪功能、社会功能、总体健康状况方面评分明显优于对照组(P<0.05),疲劳、食欲减退、恶心呕吐、便秘的症状均明显改善(P<0.05),人体功能评分明显优于对照组(P<0.001)。治疗后,试验组CA125值较对照组降低,差异显著(P<0.05)。试验组随访终点的复发死亡数较对照组低(3例vs 11例,P<0.05)。结论新益气养阴方联合化疗可改善妇科恶性肿瘤术后患者的气阴两虚证、血瘀证的中医证候,提高患者生活质量与人体功能,下调血清CA125水平,降低妇科恶性肿瘤术后复发率。
Objective To observe the efficacy of New Yiqi Yangyin Decoction combined with chemotherapy on postoperative patients with gynecological malignant tumor.Methods Sixty-four cases of gynecological malignant tumors(cervical cancer,endometrial cancer,ovarian cancer)at stageⅠ-Ⅲwere randomly divided into the test group and control group,32 cases in each group.Both groups were treated with paclitaxel plus platinum intravenous administration,with a course of 6 courses.The test group was orally treated with New Yiqi Yangyin Decoction during the chemotherapy period.The life quality of the two groups during chemotherapy was evaluated by Karnofsky scale and QLQ-C30 scale of European Organization for Research and Treatment of Cancer(EORTC).Meanwhile,the changes of Chinese medical syndrome score,serum CA125 and recurrence were observed in both groups.Results After treatment,the syndrome score of deficiency of both qi and yin in the test group was decreased compared with the control group(P<0.001),and the score of blood stasis syndrome was decreased(P<0.05).After treatment,the scores of physical function,emotional function,social function and general health in the test group were significantly better than those in the control group(P<0.05),the symptoms of fatigue,anorexia,nausea and vomiting,constipation were significantly improved(P<0.05),and the score of body function in the test group was significantly better than that in the control group(P<0.001).After treatment,the CA125 level of the test group was lower than that of the control group,with statistically significant difference(P<0.05).At the end of follow-up,the number of deaths in the test group was lower than that in the control group(3 cases vs 11 cases,P<0.05).Conclusion New Yiqi Yangyin Decoction combined with chemotherapy can improve the syndrome of deficiency of both qi and yin and the syndrome of blood stasis in postoperative patients suffering from gynecological malignant tumor,improve the life quality and body function of patients,decrease the level of serum CA125,and reduce the postoperative recurrence of gynecological malignant tumor.
