摘要
目的探讨甲强龙结合多索茶碱方案对慢性阻塞性肺疾病急性加重期(AECOPD)患者的治疗效果。方法将110例AECOPD患者按随机双盲法分为两组,对所有患者进行相同的基础治疗方案时,对照组采用多索茶碱治疗,观察组在对照组相同的治疗方案上采用甲强龙治疗;对比两组治疗前后肺通气功能、血清炎症表达等。结果治疗1周后:观察组的肺通气功能高于对照组,血清超敏C反应蛋白(hs-CRP)、白细胞介素(IL)-17、基质金属蛋白酶-9(MMP-9)表达低于对照组,而血氧分析改善程度高于对照组(P<0.05);观察组COPD评估测试(CAT)评分(15.43±2.48)分低于对照组的(21.73±3.34)分,差异明显(P<0.05)。两组治疗不良反应发生情况比较(P>0.05)。两组出院后3个月未因COPD急性发作而住院治疗,无复发情况。结论甲强龙结合多索茶碱可明显改善AECOPD患者肺通气功能,降低血清炎性因子表达,值得临床应用。
Objective:to evaluate the efficacy of methylprednisolone combined with doxofylline in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:110 AECOPD patients were randomly divided into two groups using a double-blind method,,the observation group was treated with methylprednisolone in the same treatment plan as the control group,and the pulmonary ventilation function and serum inflammatory expression were compared between the two groups before and after treatment.Results:after one week of treatment,the pulmonary ventilation function in the observation group was higher than that in the control group,and the serum levels of hs-CRP,IL-17 and Mmp-9 in the observation group were lower than those in the control group,while the serum levels of hs-CRP,IL-17 and MMP-9 in the interleukin group were lower than those in the control group,the improvement degree of blood oxygen analysis was higher than that of the control group(p<0.05),and the score of COPD assessment(Cat)in the Observation Group(15.43±2.48)was lower than that of the control group(21.73.34),the difference was significant(p<0.05).The incidence of adverse reactions in two groups was compared(P>0.05).Two groups were discharged without hospitalization for acute exacerbation of COPD within 3 months,and there was no recurrence.Conclusion:methylprednisolone combined with doxofylline can improve pulmonary ventilation function and decrease the expression of serum inflammatory factors in patients with AECOPD,which is worthy of clinical application.
