3D打印骶部模型在骶神经调节术中的运用

摘要 目的:探讨3D打印骶部模型在骶神经调节术中的运用。方法:2021年1月到2023年4月,25例骶神经调控术患者随机分为观察组(12例)和对照组(13例)。观察组术前根据影像检查进行三维重建和3D打印造模,制备出仿真骶骨、骶孔和外周软组织的模型,指导术中DSA引导下骶孔穿刺;观察组常规DSA引导下骶孔穿刺。计量资料采用t检验或Mann-Whitney U检验,计数资料采用χ^(2)检验确切概率法,比较两组患者的术前准备时间、术中透视次数、穿刺针数、手术时间、术中及术后伤口疼痛评分、体验期起效电压、永久刺激器植入率等指标。结果:两组患者性别、年龄、BMI、骶尾椎变异等一般资料比较差异无统计学意义。观察组与对照组比较,术中透视次数分别为(13.25±3.01)次与(18.84±5.01)次(P<0.05),经皮肤穿刺次数观察组与对照组分别为(7.08±2.35)次与(10.07±2.92)次(P<0.05),穿刺时间观察组与对照组分别为(38.25±13.95)分钟与(54.15±12.94)分钟(P<0.05),术中疼痛评分分别为(3.16±1.02)分与(4.07±0.75)分(P<0.05)。两组术前准备时间、体验阶段起效电压、永久刺激器植入率、术后疼痛评分、术中术后不良反应的差异无统计学意义。结论:3D打印骶部模型有助于减少骶神经调节术中穿刺针数和术中疼痛,安全有效。 Objective:To explore the application of 3D printed sacral model in sacral Neuromodulation(SNM).Methods:From January 2021 to April 2023,25 patients undergoing SNM were randomly divided into observation group(12 cases)and control group(13 cases).For the observation group,three-dimensional reconstruction and 3D printing modeling were performed according to image examination before operation.A 3D printing modeling with sacrum,sacral foramen and peripheral soft tissue was used to mimic sacral foramen puncture before operation;The observation group underwent routine DSA guided sacral foramen puncture.Preparation time,number of intraoperative fluoroscopy,total number of foramen needle skin punctures,surgical time,intraoperative and postoperative wound pain scores,onset voltage of experience period,the implantable pulse generator(IPG)implantation rates were compared between the two groups.Results:There was no statistically difference in gender,age,BMI,sacral anomalies and urodynamic conclusion between the two groups.The number of intraoperative fluoroscopy was(13.25±3.01)in the observation group and(18.84±5.01)in the control group(P<0.05).Total number of foramen needle skin punctures was(7.08±2.35)and(10.07±2.92),respectively(P<0.05).Time of punctures was(38.25±13.95)min and(54.15±12.94)min,respectively(P<0.05).Intraoperative wound pain scores was(3.16±1.02)and(4.07±0.75),respectively(P<0.05).Preparation time,onset voltage of experience period,IPG implantation rateshad no statistics difference(P>0.05).Conclusion:The 3D printing of the sacral model with soft tissue completed by the doctor team independently reduces the number of puncture needles and Intraoperative wound pain scores,which is safe and effective.
