摘要
目的探讨血清长链非编码RNA结直肠肿瘤差异表达基因(lncRNA CRNDE)、微小RNA-205-5p(miR-205-5p)表达水平在冠状动脉粥样硬化性心脏病(简称冠心病)诊断中的临床价值。方法收集2018年1月-2021年12月上海中医药大学附属普陀医院收治的300例冠心病患者为冠心病组,根据患者冠状动脉造影结果将其分为单支病变组(n=106)、双支病变组(n=115)和三支病变组(n=79);同期纳入286名排除冠心病及其他系统疾病的健康体检者为对照组。采用实时荧光定量PCR(qRT-PCR)法检测血清lncRNA CRNDE、miR-205-5p水平;采用Pearson相关性分析lncRNA CRNDE、miR-205-5p与血清指标的相关性;采用受试者工作特征(ROC)曲线分析血清lncRNA CRNDE、miR-205-5p水平对冠心病的诊断价值;采用logistic回归分析冠心病发病的影响因素。结果冠心病组lncRNA CRNDE、高密度脂蛋白胆固醇(HDL-C)水平显著低于对照组,miR-205-5p、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平显著高于对照组,差异均有统计学意义(t=11.272、21.722、14.667、11.439、13.571、7.147,P均<0.05)。冠心病患者血清lncRNA CRNDE水平与TC、TG、LDL-C成负相关,与HDL-C成正相关(r=-0.486、-0.492、-0.510、0.504,P均<0.05);miR-205-5p水平与TC、TG、LDL-C成正相关,与HDL-C成负相关(r=0.497、0.494、0.490、-0.509,P均<0.05)。三支病变组血清lncRNA CRNDE水平显著低于双支病变组和单支病变组,miR-205-5p水平显著高于双支病变组和单支病变组;双支病变组血清lncRNA CRNDE水平显著低于单支病变组,miR-205-5p水平显著高于单支病变组,差异均有统计学意义(P均<0.05)。血清lncRNA CRNDE、miR-205-5p水平联合诊断冠心病的曲线下面积(AUC)为0.843,高于lncRNA CRNDE、miR-205-5p单独诊断的0.746、0.809,差异均有统计学意义(Z=3.674、1.717,P均<0.05)。miR-205-5p是冠心病发病的独立危险因素(P<0.05),lncRNA CRNDE是冠心病发病的保护因素(P<0.05)。结论lncRNA CRNDE、miR-205-5p可作为冠心病诊断及疾病进展的可靠评估指标。
Objective To explore the clinical value of serum long non-coding RNA colorectal neoplasia differentially expressed gene(lncRNA CRNDE)and microRNA-205-5p(miR-205-5p)in the diagnosis of coronary heart disease.Methods A total of 300 patients with coronary heart disease in Putuo Hospital Affiliated of Shanghai University of Traditional Chinese Medicine were grouped into single-vessel lesion group(n=106),two-vessel lesion group(n=115)and three-vessel lesion group(n=79)according to the results of coronary angiography;meantime,286 healthy people excluding coronary heart disease and other systemic diseases were included as the control group.The levels of serum lncRNA CRNDE and miR-205-5p were detected by real-time fluorescence quantitative PCR(qRT-PCR);Pearson correlation was applied to test the correlation between lncRNA CRNDE,miR-205-5p and serum indicators;the diagnostic value of serum lncRNA CRNDE and miR-205-5p levels in patients with coronary heart disease was tested by the receiver operating characteristic(ROC)curve;logistic regression analysis was applied to examine the influencing factors of coronary heart disease.Results The levels of lncRNA CRNDE and high density lipoprotein cholesterol(HDL-C)in coronary heart disease group were obviously lower than those in control group;the levels of miR-205-5p,total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)were obviously higher than those in the control group;the differences were statistically significant(t=11.272,21.722,14.667,11.439,13.571,7.147,P all<0.05).In patients with coronary heart disease,serum lncRNA CRNDE was negatively correlated with TC,TG,LDL-C,and positively correlated with HDL-C(r=-0.486,-0.492,-0.510,0.504,P all<0.05);miR-205-5p was positively correlated with TC,TG,LDL-C,and negatively correlated with HDL-C(r=0.497,0.494,0.490,-0.509,P all<0.05).The level of serum lncRNA CRNDE in three-vessel lesion group was obviously lower than that in two-vessel lesion group and single-vessel lesion group,and the level of miR-205-5p was obviously higher than those in two-vessel lesion group and single-vessel lesion group;the level of lncRNA CRNDE in two-vessel lesion group was obviously lower than that in single-vessel lesion group,and the level of miR-205-5p was obviously higher than that in single-vessel lesion group,the differences were statistically significant(P all<0.05).The area under curve(AUC)for the combined diagnosis of coronary heart disease with serum lncRNA CRNDE and miR-205-5p levels was 0.843,which was higher than that of lncRNA CRNDE and miR-205-5p levels alone(0.746,0.809),the differences were statistically significant(Z=3.674,1.717,P all<0.05).Conclusion LncRNA CRNDE and miR-205-5p could be used as reliable indicators for the diagnosis and progression evaluation of coronary heart disease.
