摘要
目的探讨严重烧伤并发感染患者外周血人端粒酶RNA基因(hTERC)mRNA表达水平与患者病情的关系。方法选择南充市中心医院2018年5月-2020年5月收治的烧伤感染患者100例作为感染组,根据烧伤严重程度分为局部感染组(n=65)和烧伤脓毒症组(n=35),另选取本院收治的烧伤未发生感染患者60例纳入对照组。根据烧伤合并感染患者60 d治疗结局分为死亡组(n=17)和存活组(n=83)。所有患者均在入院后当日抽取外周静脉血,采用实时荧光定量聚合酶链式反应对目标基因mRNA相对表达水平进行检测。结果烧伤脓毒症组外周血hTERC、人端粒重复序列结合因子(TRF1)、TRF2 mRNA表达水平[(5.57±1.58)、(1.99±0.43)、(3.28±0.96)]高于局部感染组[(4.10±1.32)、(1.57±0.58)、(2.50±0.75)]及对照组[(2.07±0.85)、(0.88±0.30)、(1.50±0.53)],局部感染组hTERC、TRF1、TRF2 mRNA表达水平高于对照组,差异均有统计学意义(P均<0.05);死亡组hTERC、TRF1、TRF2mRNA表达水平[(5.89±1.44)、(1.83±0.55)、(3.40±0.76)]高于存活组[(4.80±1.20)、(1.43±0.40)、(2.58±0.68)],差异均有统计学意义(t=3.296、3.510、4.440,P均<0.05);以感染组患者伤后60 d治疗结局为因变量,以烧伤面积、hTERC、TRF1、TRF2 mRNA表达水平为连续自变量进行分析,多因素logistic回归分析结果显示烧伤面积、hTERC、TRF1、TRF2 mRNA表达水平为患者不良预后的独立影响因素(P<0.05)。结论人端粒酶RNA基因mRNA表达水平与严重烧伤并发感染患者病情严重程度及预后密切相关,其机制有待进一步探讨。
Objective To explore the relationships between the expression of human telomerase RNA component(h TERC)m RNA in the peripheral blood of severely burned patients with infection and the patient’s condition.Methods A total of100 patients with burn infection admitted to the Nanchong Central Hospital from May 2018 to May 2020 were selected as theresearch subjects.According to the severity of burns,they were divided into local infection group(n=65)and burn sepsisgroup(n=35).Another 60 cases of burn patients who were treated in our hospital without infection were included in thecontrol group.According to the 60 d treatment outcome,the burn coinfected patients were divided into death group(n=17)and survival group(n=83).All patients received peripheral venous blood transfusion on the day after admission.Real-timequantitive polymerase chain reaction(RT-PCR)was used to determine the m RNA levels of target genes.Results Thelevels of h TERC,telomeric repeat binding factor(TRF1)and TRF2 m RNA in peripheral blood of the burn sepsis group[(5.57±1.58),(1.99±0.43),(3.28±0.96)]were higher than those in the local infection group[(4.10±1.32),(1.57±0.58),(2.50±0.75)]and the control group[(2.07±0.85),(0.88±0.30),(1.50±0.53)].The levels of h TERC,TRF1 andTRF2 m RNA in the local infection group were higher than those in the control group,the differences were statistically significant(all P<0.05).The h TERC,TRF1,TRF2 m RNA level in the death group[(5.89±1.44),(1.83±0.55),(3.40±0.76)]were higher than those of the survival group[(4.80±1.20),(1.43±0.40),(2.58±0.68)],the differences werestatistically significant(t=3.296,3.510,4.440,all P<0.05).The 60-day treatment outcome of patients in the infection groupwas used as the dependent variable,and burn area,h TERC,TRF1,and TRF2 m RNA levels were used as continuousindependent variables for analysis.The results of multivariate logistics regression analysis showed burn area,h TERC,TRF1,TRF2 m RNA levels were independent factors influencing the patient’s poor prognosis(P<0.05).ConclusionHumantelomerase RNA component m RNA level was closely related to the severity and prognosis of patients with severe burnscomplicated by infection,and the mechanism needed to be further explored.
