摘要
目的分析2014-2018年济南市肺癌死亡和疾病负担及其变化趋势,为开展肺癌综合防控工作提供依据。方法收集2014-2018年济南市肺癌个案资料,计算死亡率、中国人口标化死亡率(ASMRC)、早死寿命损失年(YLL)、伤残所致寿命损失年(YLD)和伤残调整寿命年(DALY)等指标,不同性别间死亡率比较采用χ^(2)检验。通过Jionpoint回归计算年度变化百分比(APC),APC比较采用t检验。结果2014-2018年济南市肺癌死亡率和ASMRC分别为53.45/10万和37.73/10万,男性死亡率(71.05/10万)高于女性(36.12/10万),差异有统计学意义,χ^(2)=1812.20,P<0.001;女性标化死亡率随时间下降,差异有统计学意义,APC=-4.36%,t=-3.412,P=0.042;每千人口DALY、YLL和YLD分别为387.74、383.50和4.24,YLL/DALY为98.91%,YLD/DALY为1.09%,DALY率每年以2.20%的速度下降,差异有统计学意义,APC=-2.20%,t=-3.440,P=0.041;肺癌疾病负担随年龄增加而增加,60~<65岁负担最重,>35岁以上男性DALY率均高于女性。结论济南市肺癌死亡率高,YLL是济南市居民肺癌疾病负担的主要来源,尤其是高年龄段男性,应重点关注。
Objective To analyze the mortality and disease burden of lung cancer and its trends from 2014 to 2018 in Jinan,and to provide the evidence for lung cancer prevention and control.Methods Collected the data of lung cancer cases in Jinan from 2014 to 2018,and calculated the mortality rate,the Chinese population standardized mortality rate(ASMRC),the life lost years of premature death(YLL),the life lost years due to disability(YLD)and the disability-adjusted life years(DALY)indicators,the comparison of mortality between different sexes used theχ^(2)test.The annual percentage change(APC)was calculated by Jionpoint regression,and the t test was used for APC comparison.Results From 2014 to 2018,the mortality and ASMRC of lung cancer in Jinan were 53.45/10;and 37.73/10;.The morbidity of lung cancer in males(71.05/10;)was significantly higher than that in females(36.12/10;,χ^(2)=1812.20,P<0.001).The ASMRC of females showed a decreasing trend(APC=-4.36%,t=-3.412,P=0.042).The DALY,YLL and YLD of Jinan residents were 387740,383500 and 4240 person-years,YLL and YLD accounted for 98.91%and 1.09%of the total DALY.And the DALY rate showed a downward trend(APC=-2.20%,t=-3.440,P=0.041).The disease burden of lung cancer increased with age,and the disease burden at 60-<65 age group was highest.The DALY rate of lung cancer in males was higher than that in females age over 35 years.Conclusions Jinan has a high lung cancer mortality rate,and YLL is the main source of the burden of lung cancer among residents,especially men of older age groups,which should be paid attention to.
