摘要
目的探讨妊娠早期甲状腺功能减退(SCH)、甲状腺过氧化物酶抗体(TPOab)与妊娠期糖尿病(GDM)的相关性。方法纳入2018年1月—2020年1月兰溪市妇幼保健计划生育服务中心早期妊娠妇女510例。测血清TPOab、TSH,孕24~28周行75gOGTT检查。IBM SPSS分析数据。结果SCH、GDM发生率为13.92%和14.71%。妊娠早期SCH、TPOab(+)和年龄与GDM正相关(r=0.137,0.120,0.144;P=0.002,0.007,0.001);TPOab(+)与早期SCH正相关(r=0.257,P=0.000)。妊娠早期SCH/甲状腺功能正常组GDM发生率为26.76%和12.76%;TPOab(+)/(-)组GDM发生率为26.78%和13.22%。妊娠早期SCH的TPOab(+)/(-)组(A1/A2)、甲状腺功能正常的TPOab(+)/(-)组(B1/B2),四组间GDM发生率为36.36%、22.48%,20.59%,12.10%(P=0.000)。妊娠早期TPOab(+)组和SCH组发生GDM的OR为2.19和2.00(95%CI:1.082-4.431,1.058-3.788);随着年龄地增长,发生GDM的OR为1.09(95%Cl:1.039-1.138)。结论妊娠早期SCH合并TPOab(+)的妇女,GDM的早期干预和后期的筛查至关重要。
Objective To explore the relationship between subclinical hypothyroidism(SCH)and thyroid peroxidase antibody(TPOab)in early pregnancy with gestational diabetes mellitus(GDM).Methods 510 pregnant women in Lanxi City Maternal and Child Health Family Planning Service Center from January 2018 to January 2020 were included.Serum test TPOab、TSH,24~28 weeks of pregnancy 75 gOGTT.IBM SPSS analysis data.Results The incidences of SCH and GDM were 13.92%and 14.71%.The GDM positively correlated with SCH/TPOab(+)and age(r=0.137,0.120,0.144;P=0.002,0.007,0.001);TPOab(+)positively correlated with SCH also(r=0.257,P=0.000).The incidences of GDM in SCH and in euthyroid pregnant women were 26.76%and 12.76%and they were 26.78%and 13.22%in TPOab(+)and TPOab(-).The incidences of GDM were different in SCH with TPOab(+)/(-)(A1/A2)and euthyroid pregnant women with TPOab(+)/(-)(B1/B2)(36.36%VS22.48%VS20.59%VS 12.10%,P=0.000).The odds ratios were 2.19 and 2.002 in TPOab(+)and SCH.With the increase of age,the odds ratio of GDM is 1.088.Conclusion It's very important to screen GDM in pregnant women with a combination of SCH and TPOab(+).
