摘要
目的分析高龄老年精神障碍患者门诊就诊和治疗现状,探讨影响治疗依从性的指标。方法采用回顾性研究,通过回顾378例高龄老年患者(≥85岁)门诊电子病历,进行调查。调查的内容包括一般资料、就诊规律、就诊目的、精神疾病诊断、治疗依从性和药物治疗。结果家属代替患者就诊298例(78.8%);就诊规律,每1~2月复诊1次的173例(45.8%),就诊不规律181例(47.9%);首诊患者24例(6.3%),病情平稳,定期取药261例(69%);精神疾病患病率前3位依次为:痴呆、器质性抑郁障碍及器质性精神病。影响依从性的因素包括门诊类型、就诊是否规律和病情是否平稳。74.6%的痴呆患者使用抗痴呆药与精神科相关药物联合治疗。结论高龄老年门诊患者以器质性因素相关精神障碍为主,患者相信专家的权威性;经治疗后病情控制好,对疗效满意;能够遵医嘱随诊,有利于维持较好的治疗依从性。
Objective To analyze the current situation of outpatient diagnoses and treatment of elderly patients with mental disorder,and to explore the indexes affecting the compliance of treatment.Methods A retrospective study was carried out in 378 elderly patients(≥85 years old)with out-patient electronic medical records.The survey included general data,regularity,purpose,diagnosis of mental illness,treatment compliance and drug treatment.Results 298 cases(78.8%)were treated by family instead of patients,173 cases(45.8%)were re-diagnosed once every 1~2 months,181 cases(47.9%)were irregular.Twenty-four patients(6.3%)were diagnosed for the first time,261 patients(69%)took drugs regularly,and the first three were dementia,organic depression and organic psychosis.The factors influencing compliance included the type of outpatient service,the regularity of visits and the stability of the condition.74.6%of dementia patients were treated with anti-dementia drugs combined with psychiatric-related drugs.Conclusion The elderly outpatients mainly take the organic factors related mental disorders,the patients believe in the authority of experts;after treatment,the disease is well controlled,the curative effect is satisfactory;can follow up according to the doctorundefineds order,is helpful to maintain the better treatment compliance.
