摘要
【目的】探究心电图联合血清心型脂肪酸结合蛋白(heart-type fatty acid binding protein,H-FABP)、N-末端脑钠肽前体(N terminal protein B-type natriuretic peptide,NT-proBNP)检测对早期急性心肌梗死(acute myocardial infarction,AMI)的诊断价值。【方法】选择2015年11月至2017年11月本院收治的因胸痛入院的心内科患者106例,并将其分为AMI组(n=50)及非AMI组(n=56),行日本光电心电图仪检查,检测血清H-FABP、NT-proBNP水平,应用受试者工作特征曲线(receiver operator characteristic curve,ROC)评估H-FABP、NT-proBNP对AMI患者的诊断价值,并分析三者联合检测AMI的诊断价值。【结果】心电图检测显示AMI组符合率为86.00%;非AMI组符合率为91.07%,两组符合率比较差异无统计学意义(χ2=0.266,P=0.606);AMI组血清H-FABP、NT-proBNP水平均显著高于非AMI组(P<0.05);血清H-FABP、NT-proBNP水平单项诊断及联合诊断AMI的曲线下面积(areas under the curve,AUC)分别为0.887、0.928、0.980,诊断分界点分别为2.107μg/L、194.140 pg/ml,敏感性分别为88.00%、90.00%、90.00%,特异性分别为90.06%、92.86%、93.16%;二者联合和三者联合诊断AMI的敏感度、特异度、准确度均高于单项指标检测,且三者联合检测高于二者联合。【结论】心电图及血清H-FABP、NT-proBNP对早期AMI均具有诊断价值,且三者联合诊断价值更高。
【Objective】To explore the diagnostic value of electrocardiogram combined with serum heart-type fatty acid binding protein(H-FABP)and N terminal protein B-type natriuretic peptide(NT-pro BNP)in early acute myocardial infarction(AMI).【Methods】A total of 106 patients with chest pain admitted to our hospital were selected from November 2015 to November 2017.They were divided into AMI group(n=50)and non-AMI group(n=56)and examined by Japanese photoelectric electrocardiograph.The levels of serum H-FABP and NT-pro BNP were detected,and their diagnostic values in AMI patients were evaluated by receiver operator characteristic curve(ROC).Then the diagnostic value of combined detection was analyzed.【Results】The results of electrocardiogram showed that the coincidence rate was 86.00%in the AMI group and 91.07%in the non-AMI group,and there was no significant difference between the two groups(χ2=0.266,P=0.606).The levels of serum H-FABP and NT-pro BNP in the AMI group were significantly higher than those in the non-AMI group(P<0.05).The areas under the curve(AUC)of serum H-FABP and NT-pro BNP levels in single diagnosis and combined diagnosis of AMI were respectively 0.887,0.928 and 0.980,and the diagnostic boundary points were respectively 2.107μg/L and 194.140 pg/ml.The sensitivities were 88.00%,90.00%and 90.00%,and the specificities were 90.06%,92.86%and 93.16%.The sensitivity,specificity and accuracy of the two and three combined detections were higher than those of the single index detection,and the indexes of the three combined detection was higher than those of the two combined detection.【Conclusion】Electrocardiogram combined with serum H-FABP and NT-pro BNP is of diagnostic values in early AMI,and the diagnostic value of the three combined detection is higher.
