摘要
目的:探究医用灌注吸引平台在经尿道输尿管软镜手术中的应用价值及安全性。方法:选取2021年1月至2022年10月我院上尿路结石患者70例,采用随机数字表法分为对照组和研究组,每组35例。对照组行常规输尿管软镜碎石术(FURL)治疗,研究组行结合医用灌注吸引平台的FURL治疗。比较两组患者围手术期指标、取石效果、应激反应指标、肾脏血流动力学指标、尿控指标和并发症发生率。结果:两组患者年龄、结石部位、性别、结石最大径、病程、结石类型差异无统计学意义(P>0.05)。研究组相比对照组手术时间、术后膀胱冲洗时间、住院时间短,血红蛋白降低量低,差异具有统计学意义(P<0.05)。研究组较对照组一期清石率[91.43%(32/35)vs.71.43%(25/35)]、二期清石率[100.00%(35/35)vs.82.86%(29/35)]高,差异具有统计学意义(χ^(2)=4.629、P=0.031;χ^(2)=4.557,P=0.033)。研究组相比对照组并发症发生率低[5.71%(2/35)vs.22.86%(8/35)],差异具有统计学意义(P<0.05)。结论:上尿路结石患者FURL术中采用医用灌注吸引平台能明显提高结石清除率,降低并发症发生率。
Objective:To investigate the value and safety of the medical perfusion suction platform used in transnatural flexible ureteroscopic lithotripsy.Methods:Seventy patients with upper urinary tract stones in our hospital from January 2021 to October 2022 were selected and randomly divided into two groups of 35 cases each.The control group was treated with conventional flexible ureteroscopic lithotripsy(FURL),and the study group was treated with FURL combined with a medical perfusion suction platform.The perioperative indexes,stone extraction effect,stress response indexes,renal hemodynamic indexes,urinary control function indicators and perioperative complication rate.Results:There was no significant difference in age,site,sex,maximum stone diameter,duration of illness,and stone type between the two groups(P>0.05).The differences in surgical duration,postoperative bladder lavage duration,length of hospital stay,and hemoglobin reduction were statistically significant(P<0.05).The study group had higher rates of primary[91.43%(32/35)vs.71.43%(25/35)]and secondary[100.00%(35/35)vs.82.86%(29/35)]lithotripsy compared with the control group,and the difference was statistically significant(χ^(2)=4.629,P=0.031;χ^(2)=4.557,P=0.033).The study group had a lower rate of complications compared with the control group[5.71%(2/35)vs.22.86%(8/35)],with a statistically significant difference(P<0.05).Conclusion:The use of medical perfusion suction platform during FURL in patients with upper urinary tract stones can significantly improve the stone removal rate,reduce the complication rate.
