基于数据挖掘总结小儿肺炎支原体感染引起咳嗽的中医辨治规律 被引量:2

摘要 目的:基于数据挖掘,总结小儿支原体感染引起咳嗽的中医证治规律。方法:在中国知网、万方、维普全文知识数据库中,检索2002—2019年中医药辨治小儿肺炎支原体感染引起咳嗽的文献,通过对治疗方式的筛选、清洗、降噪及使用频率分析,利用IBM SPSS Modeler系统进行关联分析。结果:通过数据分析发现,小儿肺炎支原体感染咳嗽以热证、里证居多,多伴有夹痰,以痰热闭肺型最多,治疗上多以麻杏石甘汤加减以清肺化痰、宣肺开闭;表证、虚证较少,表证以风热犯肺型为主,在选用方药上,多以止嗽散加减,风热显著选加银翘、桑菊类,风寒显著选加麻杏剂;表证未解里证已成,多以麻杏石甘汤据表里寒热情况进行加减化裁;虚证主要是阴虚肺热、肺脾气虚,分别以沙参麦冬汤和六君子汤加减治疗。结论:根据肺炎支原体感染导致咳嗽的主症、舌脉、发热情况,能挖掘其证型、治法、方剂、中药规律,且能进行可视化展示,不仅能客观反映临床处方用药规律,还可为中医诊疗方案的制定提供参考。 Objective:Based on data mining,to summarize the TCM syndrome differentiation and treatment rules of cough caused by my-coplasma infection in children.Methods:Relevant literatures on TCM diagnosis and treatment of cough caused by mycoplasma infection were retrieved and downloaded from CNKI,Wanfang and VIP databases.Through screening,cleaning,noise reduction and frequency analysis,correlation analysis was conducted using IBM SPSS Modeler system.Results:Through data analysis,it is found that the cough caused by mycoplasma infection in children is mostly characterized by heat syndrome and interior syndrome,accompanied by phlegm re-tention,with phlegm heat blocking lung type being the most common type.Maxing Shigan Decoction is mostly used for clearing lung heat,eliminating phlegm and opening and closing lung.There are fewer exterior syndrome and deficiency syndrome,and wind-heat is more common in exterior syndrome.In the selection of prescription drugs,Zhisou Powder is used for corresponding addition and subtraction.Yinqiao and Sangju are used for those with obvious wind-heat and Maxing is used for those with obvious wind-cold.If the exterior syn-drome is not resolved and the interior syndrome is established,most of them are flavored with Maxing Shigan Decoction according to the cold and heat of the exterior syndrome.Deficiency syndrome is mainly due to deficiency of yin and lung heat and deficiency of lung and spleen qi,which are treated by modified Shashen Maidong Decoction and Liujunzi Decoction respectively.Conclusion:According to the main symptoms,tongue pulse and fever of cough caused by mycoplasma infection,the syndrome types,treatment methods,prescriptions and laws of traditional Chinese medicine can be excavated and visualized,which can not only objectively reflect the laws of clinical pre-scription and medication,but also provide reference for the formulation of relevant TCM diagnosis and treatment plans.
